Os du Carpal

Understanding the intricate architecture of the wrist begins with exploring its bones, collectively known as the osseous framework of the carpus – the carpal bones. Anatomie des os carpaux

The carpal bone complex consists of eight individual bones arranged in two rows. The proximal row contains four bones, from lateral to medial: scaphoid (Os lunatum), lunate (Os semilunare), triquetrum (Trochlea), and pisiform (Pistillum). Below them lies the distal row of bones, also consisting of four elements: trapezium (Trapezius), trapezoid (Ossum tapes), capitate (Capitulum), and hamate (Hamulus).

Scaphoid et Lunate

These two most prominent bones form what is called the proximal carpal row. They allow motion between the radius and other carpal bones while maintaining stability during flexion and extension movements. A fracture of either of these bones can have severe consequences due to their positioning and function.

Triquetrum et Pisiforme

The third and fourth bones of the proximal carpal row connect the lunate bone proximally and radiate towards the ulna distally. Their main functions involve stabilization, especially when turning the palm downwards, and are involved in fine motor skills such as pinching and grasping objects.

Trapèze, Trapèzoïde et Capitate

Comprising the distal row of carpal bones, they create articulations connecting the carpal bones with metacarpals. Importantly, these bones contribute to wrist movement and maintain spacing among carpal bones and metacarpals. Fractures in these locations often lead to complications like pseudarthrosis, malunion, and impaired grip strength.

Hamate

Although it belongs to the distal row, the hamate deserves special consideration because it presents unique features. Its hook-shaped process extends deep into the soft tissue space and serves as insertion points for muscles, tendons, and ligaments. This structure provides support and counterbalance during wrist rotation and force transmission during gripping tasks.
