Fetal Monitoring

Fetal monitoring is a critical aspect of intrapartum care, which involves continuous fetal heart rate evaluation during delivery. It helps determine the well-being of the unborn child throughout the process of labour. There are various methods used in fetal heart rate monitoring, including:

External Electrode

An external electrode device clips onto the mother's abdomen and records contractions on paper strips. This method provides a clear visual representation of contraction strength and duration.

Internal Scalp Electrode

This method involves insertion of an electrode into the baby's scalp through the cervix. it can be used when contractions become less detectable with the previous method.

Doppler Monitoring

A handheld ultrasound wand is moved over the maternal abdomen to transduce sound waves across the uterine wall. A good Doppler signal indicates proper positioning and function of the internal electrode.

Continuous Electronic Fetal Monitoring

In this technique, data from the fetal monitor are converted into electrical impulses and recorded electronically on paper or stored digitally. Continuous electronic fetal monitoring can reduce the risk of brain injury due to oxygen deprivation by detecting signs that require prompt intervention.

Stages of Labor

The second stage of labor begins after full dilation of the cervix and continues until birth of the baby. This stage is divided into three phases: early, active, and transition phase:

Early Phase

In this phase, the mother begins to push in a controlled manner, with contractions occurring every 2 to 3 minutes. The baby is positioned head down.

Active Phase

This phase is marked by increased pushing intensity and frequency, with contractions occurring every 1 to 2 minutes. During this phase, the baby's heart rate should remain within normal limits.

Transition Phase

This final phase occurs when the mother's efforts are focused on pushing the fetal head through the birth canal. It may last for several minutes to an hour, depending on factors such as how dilated the cervix is. This phase can be challenging for both the mother and nurse due to intense pain and fatigue.

Intranatal nursing focuses on providing support and guidance during this critical period of labor. Nurses ensure that proper monitoring techniques are used to assess both the progression of labor and the wellbeing of the unborn child.
