Newborn Priorities

- Temperature regulation is vital. Maintaining optimal oxygenation.
- Prevention of infection is crucial.
- Establishing an infant-parent relationship should be prioritized.
Initiation and Maintenance of Respirations
- Respiratory acidosis and severe complications can result from breathing problems.
- Factors that can predispose newborns to respiratory issues include low birth weight, maternal diabetes, premature rupture of membranes, maternal narcotic use and anomalies of the chest, heart, or respiratory tract.
- Proper airway management is essential.
- Suctioning and avoiding unnecessary stimulation is vital.
Resuscitation Process
- Airway management involves use of bulb syringe for mucus or fluid removal if needed.
- Avoid rubbing for meconium-stained fluid; gently suction.
- Keep the newborn dry.
- Limited suctioning to 10 seconds to prevent trauma.
- Lung expansion is critical. Crying helps with lung expansion.
- Respiratory support with oxygen (100%) via bag and mask may be needed.
- Monitor oxygen levels.
- Drug therapy (naloxone) may be administered if respiratory depression is from maternal narcotics.
Maintaining Fluid and Electrolyte Balance
- Dehydration from rapid respirations.
- Potential for hypoglycemia with energy expenditure during resuscitation.
- Fluid replacement with Ringer's lactate or 10% dextrose in water.
- Monitor fluid intake for potential overload.
Regulating Temperature
- Maintaining optimal oxygenation and minimizing metabolic demand.
- Chilling increases oxygen consumption.
- Prolonged vasoconstriction from chilling also reduces respiration and releases acid, rising risk of kernicterus.
- Dry the newborn, cover with a cap, and place under a pre-warmed radiant warmer or incubator.
- Ensure the axillary temperature is maintained at 97.8°F (36.5°C).
