Health History and SOAP Documentation
- SOAP stands for subjective, objective, assessment, and plan.
- Old carts (onset, location, duration, characteristics, alleviating/aggravating factors, radiation or relieving pain, timing, severity) are important elements in documenting patient history.
- HPI (History of Present Illness) is a crucial component of the patient's history.
- PQRST (precipitation, quality, radiation, severity, timing) is an acronym used to remember key components of patient symptoms.
- Client outcomes (character, location, impact, expectation, neglect, timing, other, understanding/beliefs, treatment, complementary, options, modulating, exposure, spirituality) are important considerations.
- SOAP notes include subjective data (provided by the patient, including ROS - review of systems), and objective data (assessment by the practitioner).
- Past medical history is not relevant to the current visit, even if the patient has a comorbidity
Common Geriatric Skin Lesions
- Xerosis is impaired keratinocyte formation leading to abnormal epidermal cell turnover, commonly causing itching in the elderly.
- Stasis dermatitis is dry, itchy skin affecting the lower extremities, often associated with vascular changes (varicose veins) and edema. Inflamed skin may become ulcerated.
Common Geriatric Skin Tumors
- Seborrheic keratosis appears stuck-on, varying in color and dryness, often waxy and pebbly. Can be mistaken for other conditions like skin cancers. Should be evaluated properly.
- Cherry angioma (senile angioma) are smooth, firm, deep red lesions, often increasing in number with age.
