Study Notes
Hand and Finger Injuries
- Hand and finger injuries are a recurring safety concern
- Two recent incidents are highlighted
- Incident 1: A worker crushed their small finger while moving a moonpool door ram. The ram moved unexpectedly, despite a toolbox talk on manual handling. No-one stopped to assess the risk of the ram moving. Resulting in a crushed finger.
- Incident 2: A worker cut their hand on a ceiling plate. They weren't wearing gloves.
- Lessons: Stop and think before starting work. Be aware of your surroundings. Use appropriate personal protective equipment (PPE).

- Findings:
- Catastrophic failure of one generator
- Damage to a second generator
- Hi-Fog system was effective.
- Fire suits and PPE were effective
- Effective response from offshore and onboard teams.
- Lessons: A thermal imaging camera is potentially helpful. Ensure all personnel are familiar with emergency procedures.
BSEE: Welding and Burning Hazards
- BSEE discovered several critical issues with welding and burning operations in the Gulf of Mexico.
- Hot slag from a cutting torch pierced the underside blanket barrier, causing a fire.
