Glomerular Diseases

  • Glomerulopathy is a broad term that includes all glomerular diseases, which can be primary or secondary (part of a systemic disease).

Classification of Glomerulopathies

  • Glomerulonephritis: conditions involving inflammation of the glomerulus, which can be acute or chronic.
    • Characterized by asymptomatic hematuria, nephritic syndrome, and rapidly progressive glomerulonephritis (RPGN).
  • Nephrotic glomerulopathy: conditions generally due to glomerular epithelial cell injury.
    • Characterized by asymptomatic proteinuria and nephrotic syndrome.

Chronic Kidney Disease

  • Defined as the progressive loss of renal function, which can be caused by glomerular, tubular, or interstitial disease.
  • The final common pathway is usually glomerulosclerosis, characterized by segmental or global collapse with closure of the capillary lumens, making it impossible to filter plasma.

Types of Glomerular Lesions

  • Focal: involves < 50% of glomeruli on light microscopy.
  • Diffuse: involves > 50% of glomeruli on light microscopy.
  • Segmental: involves only part of the glomerular tuft.
  • Global: involves the entire glomerular tuft.
  • Proliferative: characterized by an increased number of cells in the glomerulus, usually inflammatory cells.
  • Membranous: characterized by thickening of the glomerular basement membrane.
  • Crescent: characterized by accumulation of cells and proliferating parietal epithelial cells within Bowman's space, associated with severe disease.
  • Membranoproliferative: characterized by a thickened glomerular basement membrane and proliferation.
