- On March 7, 2023, a gas leakage was reported at well site #16 at Mari Field Daharki due to a hole in the thread protection plug of the Access Fitting Assembly.
- An investigation team was constituted to identify the root cause and propose recommendations to avoid reoccurrence.
- Production and C&I teams were mobilized to assess the extent of leakage and perform rectification.
- Line depressurization was initiated by shutting off Wells#16 and 97 and isolating the 8” line from the HRL network.
- Rectification of the leaked Access Fitting Assembly was concluded by the C&I team at 1600 hrs and pipeline loop was re-pressurized, and production resumed at 1700 hrs.
- The cellar uplifting activity at Mari Field Daharki is currently being executed by the ED&P team, and uplifting activity has been carried out at 57/87 locations.
- Gas leakage detection survey has been started at all corrosion monitoring access fittings recently installed at uplifted cellar locations.
- The primary cause of gas escape was found to be a loosened bore seal nut, which was not properly tightened during the installation of the access fitting assembly.
- Roles and responsibilities, as well as communication protocols, were not distinctly defined during the installation of access fitting assemblies.
- Recommendations include checking all corrosion monitoring access fittings assemblies at uplifted cellar locations for leakage, clearly defining roles and responsibilities for installation of access fitting assemblies, and developing or updating work instructions for QA/QC of newly installed access fittings.
- The permit shall be closed out after the punch listing has been conducted by the Maintenance (Static/C&I/E&I), E&C, and Production teams.
- All dossiers must be handed over to the Operations Team for an effective punch listing of the already uplifted cellars, as well as ongoing and upcoming cellars.
