- Cumulative Discipline entails one previous infraction in the same alphabetical grouping, or two in different groupings.

- Inquiries are completed on the Customer Inquiry Log (does not require Customer Complaint Checklist).
- Customer complaint form is initiated by LTs or higher. If BC unavailable, contact DC verbally.
- DC contacts Admin Secretary or FOO for a Customer Complaint Tracking Number.
- DC may delegate fact finding to BC at the battalion level.
- The Customer Complaint Checklist is a guide and coversheet and a procedural document.
- Using the Customer Complaint and Inquiry Field Guide, ensure all actions are summarized in the “actions taken” section of the Customer Complaint Form.
- DC changes status from open to submit, then assigns to the next level supervisor.

- If injury involves PPE, collected by the BC and coordinate with Training and Safety Division.
- Injury with no treatment, the Supervisor-BC-DC-Training.
- Employee should be seen at the OHC within 48 hours.
- State bureau notified if: firefighter hospitalized more than 24 hours, LODD, or serious injury involving equipment failure. Contact should be made within 4 hours and done by the FOO with authorization from the appropriate Deputy Chief.
- Medical exam and fitness assessment if out of work greater than 6 months, may be held on days two weeks or longer to complete training and certifications.
- EAP Management Referral: BC or higher. Wellness Coordinator notified then notifies EAP coordinator and informs Deputy Chief of Operations.
