    Study Notes

    Fire Ground Accountability

    • The Hoffman Estates Fire Department uses the PASSPORT system for firefighter accountability at all incidents that involve mutual aid or MABAS Box Alarm response or greater.
    • The PASSPORT system is a plastic card with crew members’ names attached, which are turned into a fireground manager.
    • The PASSPORT system includes a 2" x 4" plastic card with the company designation, apparatus number, and the department's MABAS designation (HFES).
    • Each firefighter is responsible for maintaining three name tags which are affixed to Velcro strips on the under side of their helmet.
    • The engineer's name tag is inverted on both the White and Red PASSPORTS.
    • All engines, towers, ambulances, staff vehicles, and specialized apparatus are equipped with a status board, and all staff vehicles are equipped with PASSPORT makeup kits.
    • Each firefighter must have a helmet shield designating their apparatus assignment attached to their helmet.

    Tactical Benchmarks

    • The Personnel Accountability Report (PAR) involves a roll call of personnel assigned, and is conducted whenever a firefighter is reported missing or trapped, there is a change from offensive to defensive operations, there is a sudden hazardous event, it is reported fire under control, or the incident commander determines it is necessary.

    Accountability Officers

    • The exterior command post serves as the initial accountability location.
    • Once divisions are established, companies entering the incident will deliver their PASSPORTS to the accountability location closest to their point of entry before entering the hazard zone.
    • Accountability Officers will report to their assigned Division Supervisors to manage accountability for their division.

    Accountability Division

    • The Incident Commander (IC) should implement an Accountability Division when the incident escalates to the level where Accountability Officers are assigned.
    • The Accountability Division Supervisor’s responsibilities include developing a plan to track and account for all personnel working in the hazard zone, implementing Accountability Officers in each division, initiating PARs upon benchmarks or as needed, and providing progress reports to Command/Planning Section.

    Rules of Thumb

    • The PASSPORT card should never enter the hazard zone.
    • Crews must turn in their PASSPORT cards before entering the hazard zone and must retrieve their PASSPORTS upon exiting from the hazard zone.

    Multi-Story/Highrise

    • For multistory or high-rise incidents, crews reporting to the building will deliver their PASSPORTS to the Lobby Division, which is responsible for collecting PASSPORTS as soon as possible.

    Rapid Intervention Crew (RIC)

    • The Incident Commander will assign a Rapid Intervention Crew (RIC) to serve as a stand-by rescue team during all hazardous operations.

    Lost/Missing Firefighter

    • An absent member of any crew will automatically be assumed lost or trapped in the hazard zone until otherwise determined safe.
    • Company Officers must immediately report any absent members to Division Supervisors or the Incident Commander.
    • If there are reports of missing firefighters, the Incident Commander should request a Mayday Response to NWCD, request NWCD to backfill change of quarters companies, initiate an immediate roll call of all companies assigned to duty, and send the Rapid Intervention Crew (RIC) to begin a search.

    Summary of Accountability Responsibilities

    • Firefighter - responsible for staying with their crew, ensuring that their name tag is on the PASSPORT, and their helmet has the correct helmet shield.
    • Company Officer - responsible for keeping their crew intact, maintaining accurate PASSPORTS, and helmet shields for their crews.

    Hazard Zone Management

    • The first arriving officer should communicate a clear description of the incident to incoming units, including the height of the building, the occupancy type, and the location and severity of the fire or smoke.
    • The officer should also announce Command and state the direction of travel.
    • A 360-degree size-up must be conducted to assess the building's construction, identify hazards, and determine the strategy (offensive, defensive, or transitional).
    • Tactical benchmarks, such as "all clear" and "fire control," should be relayed to dispatch once completed.
    • All responding apparatus should stage until assigned by Command.
    • Companies should be assigned to address utilities, potential patients, ventilation, overhaul, ladders, monitors, and RIC/IRIC.
    • "On Deck" crews are staged with PPE, SCBA, and tools, ready to take over assignments or perform RIT functions.
    • "Recycle" crews are those who work through one cylinder of air, hydrate, change cylinders, and return to their last division or "On Deck".
    • "Rehab" refers to moving to another division and units may be reassigned out of rehab if required.
    • Sides of a structure are labeled Alpha, Bravo, Charlie, and Delta, clockwise from the front.
    • Floors are labeled using numbers (Basement, 1, 2, etc., Roof).
    • Exposures are structures or units adjacent to the main fire area.
    • A C.A.N. report is a brief briefing format that covers "Conditions," "Actions," and "Needs."
    • P.A.R. (Personnel Accountability Report) verifies that all crew members are present and accounted for.
    • A transitional attack is an exterior fire attack used to knock down visible fire before entering the structure. It is considered an offensive attack if the plan is to go interior.
    • Operations on lightweight construction require extreme caution, potentially using platforms, main ladders, or ground ladders.
    • Offensive operations are permitted when entry into a structure is possible and necessary to save lives or minimize property damage.
    • Defensive operations are implemented when the situation is not survivable and property cannot be saved, prioritizing exposure protection and establishing collapse zones and exclusion zones.
    • A back-up line reinforces an existing tactical position and should be the same size or larger than the original line.
    • A second line is deployed to another fire area of the structure as assigned by the IC.
