Treatment and Transportation of the Injured
- Officer/Supervisor must arrange for immediate first aid treatment and transportation to a medical facility for the injured person where required
- Ensure a District Chief attends at the hospital where the injured member is transported to
- Officer attending hospital must not wear bunker suit, helmet, or boots
Safety and Scene Preservation
- In case of a significant injury or incident, Officer/Supervisor in charge ensures the scene is made safe and not disturbed until released by CJHSC Management Co-Chair or designate
Investigation
- Purpose of an injury, accident, or incident investigation is to discover underlying causes to prevent future occurrence
- Individual's immediate supervisor begins the investigation
- Investigation is fact-finding, not fault-finding, and not for laying blame
- Co-operation of all TFS personnel in providing pertinent information to the Investigator(s) is critical
Investigating Significant Injuries and Significant Incidents
- All significant injuries and near-miss incidents occurring to TFS personnel need to be investigated by the CJHSC Co-Chairs
- Purpose of the investigation is to:
- Determine causes of the injury/accident
- Prepare a report describing circumstances and causes of the injury/accident
- Make recommendations for remedial actions to prevent future occurrences
- Investigation reports will be distributed to the Central JHSC Co-Chairs and the applicable Deputy Chiefs or designates
Investigator's Authority
- Investigator(s) or investigation team is authorized to:
- Examine the scene and equipment involved
- Interview witnesses
- Obtain and copy documents for examination
- Request tests and review reports to determine causes of the accident
WSIB Reporting Lost Time When Off Duty
- Employee must report a work-related injury as soon as it is recognized, even after the end of a work shift
- Employee must seek appropriate medical attention
- Employee reports the injury to the On-duty Platoon Chief, who determines if the injury is critical and ensures proper notifications
