Accident Documentation

Response and Approval Processes
- Office of Fire Investigation (OFI) must respond to all accidents leading to injuries and significant incidents; applicable to Classes 1(A), 2(A), 3(A), and 4(A).
- All Department accidents necessitate the response of a Battalion Chief (BC) and a Dispatch Data Coordinator (DDC).
Specific Situational Protocols
- If involved in a Class 4A accident, personnel cannot continue to a run if there are injuries; however, if it is determined to be a Class 4B (without injuries), personnel may proceed if the vehicle is safe.
- In accidents involving two departmental vehicles, documentation must be separate and classified as Class 1(A) or Class 1(B) based on injuries.
