Dental Charting Procedures

• Existing restorations and conditions are marked with a blue pencil.
• Conditions requiring treatment are designated with a red pencil.
• Temporary restorations are indicated using a black pencil.
• Documentation is recorded in black ink.

Specific Tooth Conditions and Charting

• For tooth #1 not clinically present, draw a blue X over the entire tooth.
• An MODBL amalgam on tooth #2 is outlined and solidly filled in blue.
• Temporary restoration on the occlusal surface of tooth #3 is outlined in black, labeled "temp."
• Tooth #4 needing a dental sealant is marked with a red "S" on the occlusal surface.
• For a porcelain fused to metal crown on tooth #5, outline in blue and indicate metal with a solid blue line on the lingual surface.

Extractions and Special Cases

• Tooth #6 requiring extraction is marked with a red "/."
• A supernumerary tooth between #6 and #7 is represented in blue.
• A porcelain veneer on tooth #7 is outlined in blue on the facial and incisal surfaces.
• An ML composite restoration on tooth #8 is outlined in blue without solid filling.

Dental Bridges and Sealants

• For an all-porcelain bridge involving teeth #9-11, outline in blue; mark missing tooth #10 with a blue X over the roots.
• Dental sealant on tooth #12 is indicated with a blue "S."

Dental Decays and Restoration Details

• An MO amalgam with recurrent decay on tooth #13 is outlined and marked in red.
• A full metal gold crown on tooth #14 receives a blue outline with diagonal lines filled in blue.
• For a DO composite on tooth #15, outline in blue without solid filling.

Additional Tooth Conditions

• Partially erupted tooth #16 is marked with "PE" in blue.
• Tooth #17 with root remnants is marked with a red X and labeled "RR."
• Outline surfaces of tooth #18 with blue diagonal lines for a three-quarter gold crown.
• For tooth #19 with a root canal and post/core, outline each canal in blue.

Further Conditions and Notes

• Tooth #20 with DO decay is filled in red to indicate affected surfaces.
• Class V buccal composite restoration on tooth #21 is outlined in blue without solid filling.
• Tooth #22's DL amalgam is colored in blue.
• A diastema between teeth #22-23 is represented with blue parallel lines.
• An implant on tooth #23 is marked with horizontal lines and outlined in blue.

Abscesses, Fractures, and Extractions

• An abscess on tooth #24 is indicated by a red circle around the root apex.
• A fracture on tooth #25 is shown with a jagged red line.
• Tooth #26 extraction is marked with a blue X.
• Tooth #27 drifting mesially is indicated with a blue arrow showing direction.
• An occlusal gold inlay on tooth #28 is outlined in blue with diagonal lines.

Restorative Notes

• An MOD amalgam with mesial and distal overhang on tooth #29 is charted in blue with red markers.
• Tooth #30 with a stainless steel crown is outlined and filled in blue with "SSC" written in blue.
• Tooth #31 with MOD amalgam restoration is outlined and filled in blue.
• An impacted tooth #32 is circled entirely in red.
