- For tooth #1 not clinically present, draw a blue X over the entire tooth.
- An MODBL amalgam on tooth #2 is outlined and solidly filled in blue.
- Temporary restoration on the occlusal surface of tooth #3 is outlined in black, labeled "temp."
- Tooth #4 needing a dental sealant is marked with a red "S" on the occlusal surface.
- For a porcelain fused to metal crown on tooth #5, outline in blue and indicate metal with a solid blue line on the lingual surface.
Extractions and Special Cases
