Defibrillation Overview
- Defibrillators are devices that apply an electric charge to the heart to restore a normal heartbeat
- Used when the heart rhythm stops due to cardiac arrest (SCA)
- This can help the heart start beating again
Defibrillation Types
- Biphasic wave form: 120-200 J
- Monophasic wave form: 360 J
- AED-device specific
Defibrillation Procedure Steps
-
Safety:
- Remove oxygen delivery devices if the patient is not intubated
- If intubated, leave the bag valve resuscitator attached to the patient or remove it.
- Use self-adhesive defibrillation pads if available.
- Do not place pads over pacemakers.
- Remove transdermal patches.
-
Procedure:
- Place sternal paddle over right of the sternum below the clavicle
- Place apical paddle in the mid-axillary line in the 5th intercostal space
- Turn on the defibrillator
- Charge the defibrillator to 200J or 360J
- Tell all other rescuers to stand clear - "ARE YOU CLEAR?"
- Visually check that everyone is clear
- Ensure that you are not touching the patient or the bed - "I AM CLEAR"
-
Deliver shock
-
Restart CPR without checking pulse
Automatic External Defibrillator (AED) Steps
- Turn on AED
- Attach electrode pads
- Place electrodes in the standard positions, matching that of a manual device
- Follow voice commands
- Ensure no one is touching the patient
- Press the shock button
