Study Notes
Nursing Theories and Models
- A discipline's central ideas and phenomena are identified through conceptual frameworks, which are sets of related ideas that follow a pattern.
- Nursing theories explain, describe, and predict nursing practice outcomes, and are used in education, research, and practice.
- Theories are logical and orderly, describing relationships between concepts, and are used to identify gaps and problems in nursing practice.
System's Theory
- A system consists of matter, energy, and communication, and is interrelated, meaning a change in one part affects another part.
- Open systems interact with their environment, maintaining balance through feedback for survival.
- Closed systems do not interact with their environment and do not exist outside of a laboratory setting.
Neuman Systems Model
- Developed by Betty Neuman, the model is based on an individual's relationship to stress and is a holistic, open-systems approach.
- The model incorporates health as a continuum from wellness to illness to death, and considers the client (person, family, group, community) and environment.
- Nursing is focused on prevention as an intervention, and system stability is the goal.
Client Variables
- Physical/Physiological Variable: bodily structure and function
- Psychological Variable: mental processes and relationships
- Sociocultural Variable: social and cultural expectations and activities
- Developmental Variable: age-related developmental processes and activities
- Spiritual Variable: spiritual beliefs and influences
Environment
- Intrapersonal Stressors: inside the client/client system (e.g., infection, medical or psychological diagnosis)
- Interpersonal Stressors: outside the client/client system but in close proximity (e.g., role expectations)
- Extrapersonal Stressors: outside the client/client system at a distal range (e.g., financial concerns)
Client System Diagram
- The client system consists of a core surrounded by a Flexible Line of Defense, Normal Line of Defense, and Lines of Resistance
- The Flexible Line of Defense is the outer boundary, preventing stressor invasion and protecting the Normal Line of Defense
- The Normal Line of Defense is the usual level of health and wellness, responding to coping or environmental changes
- Lines of Resistance are activated when the stressor invades the Normal Line of Defense, attempting to stabilize the client system
Health and System Stability
- Health is a stable state of the client system
- The Neuman Systems Model aims to maintain or reconstitute system stability
