Comprehensive Geriatric Assessment (CGA)
- CGA is an interdisciplinary process assessing older adults using a biopsychosocial functional model
- Three underlying principles: physical, psychological, and socioeconomic factors
- Focus on functional abilities central to evaluation
- Purpose: To prevent functional decline and improve care for older adults
Tools Used in CGA
- Physical: Physical examination, nutritional assessment (BMI, MNA)
- Functional: Barthel's Index, Katz Index of Independence in ADLs, Lawton Instrumental Activities of Daily Living Scale, Time Up and Go (TUG)
- Socioeconomic: Financial situation, family APGAR, housing
- Psychological: Cognitive assessment (MMSE, Mini-Cog, MoCA), mood and emotion
- Medical: Review of systems, comorbidities
- Culturally sensitive: considering patients cultural and social backgrounds, avoiding stereotypes
Special Considerations for CGA
- Atypical illness presentation: altered presentation of illness in older adults
- Interrelationship between physical, psychosocial aspects of aging:
- Health conditions and disability affects functional status
- Individualized assessment: Tailoring the CGA to the individual older adult
The Interviewer
- Cultural sensitivity is critical: Be respectful and understanding, avoid stereotyping, learn about patient's culture's beliefs and practices.
- Communication is key: Use clear, concise explanations, allow time for the patient to respond, respect personal space.
- EHR use during an interview may affect communication and should be used mindfully to improve interview quality.
