Clindamycin Overview
- Clindamycin is an antibiotic available in different formulations including hydrochloride, palmitate hydrochloride, and phosphate.
Trade Names
- Commonly known as Cleocin Hcl, Dalacin C, and Cleocin Pediatric.
Classification
- Classified as an antibiotic, effective in treating various bacterial infections.
Dosage Forms
- Clindamycin hydrochloride: Available in capsules of 75mg, 150mg, and 300mg.
- Clindamycin palmitate hydrochloride: Offered as granules for oral solution at 75mg/5ml.
- Clindamycin phosphate: Available in injectable infusions in D5W (300mg/50ml, 600mg/50ml, 900mg/50ml) and multiple injection forms (150mg base/ml, 300mg base/2ml, 600mg base/4ml, 900mg base/6ml).
Mechanism of Action
- Works by inhibiting bacterial protein synthesis through binding to the 50S subunit of the ribosome.
Indications
- Effective against infections caused by sensitive staphylococci, streptococci, pneumococci, and a range of anaerobic and aerobic organisms.
- Also used in the treatment of pelvic inflammatory disease.
Contraindications
- Not recommended for patients hypersensitive to clindamycin or lincomycin.
- Risk of overgrowth of non-susceptible organisms such as yeasts; monitor for signs of superinfection.
- Use cautiously in neonates and individuals with renal or hepatic impairment, asthma, or a history of gastrointestinal disease.
Adverse Effects
- Potential side effects include headache, pharyngitis, abdominal pain, bloody diarrhea, colitis, constipation, gastrointestinal upset, skin dryness, redness, burning, contact dermatitis, irritation, and rash.
Nursing Implications
- IM injections may elevate creatine kinase levels due to muscle irritation.
- Continuous monitoring of renal, hepatic, and hematopoietic function is necessary.
- Avoid using opioid antidiarrheals for drug-induced diarrhea, as they may worsen the condition.
