Chest Wall Neurology
- Dermatomes are areas of skin supplied by a single nerve root
- Intercostal nerves supply a dermatome
- Chest wall dermatomes overlap, half by the nerve above and below
- Anterior rami of each nerve supply the skin and parietal pleura
- Intercostal muscles are supplied by anterior rami of thoracic spinal nerves
- Posterior rami of each nerve supply the posterior vertebral muscles and overlying skin
- Thoracocentesis (pleural tap) or nerve block should be done in the middle of the intercostal space for safety
- Nerve block is performed by injecting near the rib above and below
- Mechano-receptors in the chest wall and respiratory muscles affect the sensation of dyspnoea
