Study Notes
Calcitonin Overview
- Prototype drug: Calcitonin-salmon (marketed as Fortical, Miracalcin).
Expected Pharmacologic Action
- Decreases bone resorption by inhibiting osteoclast activity, beneficial in treating osteoporosis.
- Increases renal calcium excretion by inhibiting tubular resorption, helping to manage calcium levels.
Therapeutic Uses
- Primarily treats postmenopausal osteoporosis (does not prevent it).
- Effective in treating Paget's disease and managing hypercalcemia resulting from hyperparathyroidism and certain cancers.
Adverse Effects
- Common side effect: Nausea; usually self-limiting.
- Nasal dryness and irritation can occur with intranasal administration; advise to alternate nostrils daily and inspect nasal mucosa for possible ulceration.
Contraindications/Precautions
- Classified as pregnancy risk category C.
- Contraindicated for individuals with hypersensitivity to calcitonin or fish protein; skin allergy testing is recommended before administration.
- Caution is advised for lactating women, children, and patients with kidney disease.
Interactions
- Concurrent use with lithium may lead to decreased serum lithium levels; close monitoring of levels is essential.
Evaluation of Effectiveness
- Effectiveness is indicated by increased bone density.
- Serum calcium levels should be maintained within the expected range of 9-10.5 mg/dL.
Administration Guidelines
- Commonly administered via nasal spray; also available through intramuscular (IM) or subcutaneous (SC) injection; rotate injection sites to prevent inflammation.
- Store the medication in an upright position.
- Alternate nostrils daily for nasal administration.
- Regularly check for Chvostek's and Trousseau's signs to detect hypocalcemia.
- Monitor bone density scans periodically.
- Encourage a diet rich in calcium and vitamin D to support treatment goals.
