  • Raynaud's phenomenon
  • Peripheral artery disease due to atherosclerosis
  • Deep vein thrombosis
  • Buerger's disease (correct)

What is the primary pathological mechanism underlying the development of Buerger's disease?

  • Venous valve incompetence leading to blood pooling
  • Spasmodic constriction of arteries in response to cold or stress
  • Accumulation of cholesterol plaques in large arteries
  • Vasculitis of small and medium arteries due to chemical exposure (correct)

A patient with Buerger's disease is prescribed nifedipine. What is the intended therapeutic effect of this medication?

  • To reduce inflammation in the arteries
  • To prevent blood clot formation
  • To decrease blood pressure
  • To dilate blood vessels, improving blood flow (correct)

Study Notes

  • Buerger's disease is an inflammatory condition that impairs circulation to the extremities (arms and legs).
  • Smoking is the key risk factor.
  • Chemicals in tobacco cause vasculitis, inflammation of small and medium arteries in the arms and legs.
  • Inflammation leads to scarring and occlusion of blood vessels.
  • Typical patient: male, 20-50 years old, who smokes.
  • Signs and symptoms:
    • Claudication (pain with activity)
    • Numbness and tingling
    • Decreased pedal pulses
    • Decreased temperature in the extremities
    • Possible cyanosis
  • If the disease progresses, total blockage of extremities blood flow causes tissue death.
  • Gangrene may occur, leading to loss of digits or amputation.
  • Diagnosis: arteriogram to visualize the inside of the arteries.
  • Treatment: avoid all tobacco products; vasodilators such as nifedipine.
  • Patient teaching:
    • Quit smoking and cease using any tobacco products.
    • Avoid extreme cold, which causes vasoconstriction and impairs blood flow.
