    Study Notes

    Breast Tissue Alterations

    • Exaggeration and distortion of normal cyclic breast changes in the menstrual cycle lead to a range of alterations.

    Nonproliferative Change

    • Characterized by an increase in fibrous stroma associated with duct dilation and formation of cysts of various sizes.
    • Most common type of alteration.

    Proliferative Change

    • Includes proliferative lesions within ductules, terminal ducts, and sometimes lobules of the breast.
    • Epithelial hyperplasias can be mild and orderly, carrying little risk of carcinoma, or florid and atypical, carrying a significantly greater risk.

    Sclerosing Adenosis

    • Less common than cysts and hyperplasia, but significant due to its deceptively similar clinical and morphologic features to those of carcinoma.
    • Characterized by marked intralobular fibrosis and proliferation of small ductules and acini.

    Fibroadenoma

    • Most common benign tumor of the female breast.
    • Thought to develop due to absolute or relative increase in estrogen activity.
    • Usually appears in young women, with peak incidence in the third decade of life.
    • Morphology: well-defined, solitary, freely movable nodule, 1-10 cm in diameter, rarely multiple tumors or exceeding 10 cm in diameter (giant fibroadenoma).
    • Histology: loose fibroblastic stroma containing ductlike, epithelium-lined spaces of various forms and sizes, with single or multiple layers of regular cells and a well-defined, intact basement membrane.
    • Surrounded by a fibrous capsule.

    Fibroadenoma Variations

    • Pericanalicular fibroadenoma: ductal spaces are open, round to oval, and fairly regular.
    • Intracanalicular fibroadenoma: ductal spaces are compressed by extensive proliferation of the stroma, appearing as slits or irregular, star-shaped structures on cross-section.
