Flashcards

What is the Basic Periodontal Examination (BPE)?

A simple screening tool to estimate the level of further periodontal examination needed.

What does the BPE assess?

Pocket depths, calculus, overhangs, and bleeding on probing.

Describe the BPE probe.

It has a ball tip and is marked in sections to represent each sextant of the mouth.

What are the sextants of mouth when using the BPE probe?

Upper Right (UR), Upper Anterior (UA), Upper Left (UL), Lower Right (LR), Lower Anterior (LA), Lower Left (LL).

Study Notes

Basic Periodontal Examination

  • The Basic Periodontal Examination (BPE) was developed by the British Society of Periodontology in 1986 and is a simple screening tool that estimates the level of further examination needed.
  • BPE is a minimum standard of care for initial periodontal assessment. It is not a treatment outcome measure or a diagnostic tool.
  • The BPE is used to assess pocket depths, calculus, overhangs and bleeding on probing.
  • The BPE should be used for screening only, not for diagnosis.

BPE Probe

  • The BPE Probe is a WHO probe with a ball tip designed for periodontal examination.
  • The probe is divided into six sections, each representing a sextant of the mouth:
    • UR (17-14)
    • UA (13-23)
    • UL (24 – 27)
    • LR (47-44)
    • LA (43-33)
    • LL (34 – 37)
  • Each sextant must have at least two teeth for a BPE score and must be recorded, even if no teeth are present.

BPE Codes

  • Code 0 is assigned when there are no pockets deeper than 3.5 mm, no calculus or overhangs, and no bleeding on probing.
  • Code 1 is assigned when there are no pockets deeper than 3.5 mm, no calculus or overhangs, but bleeding occurs after probing.
  • Code 2 is assigned when there are no pockets deeper than 3.5 mm, but calculus or overhangs are present.
  • Code 3 is assigned when there are pockets between 3.5 and 5.5 mm deep.
  • Code 4 is assigned when there are pockets deeper than 5.5 mm.

BPE Limitations

  • The BPE does not provide a definitive diagnosis, only a level of periodontal health.
  • The BPE is not a treatment outcome measure and should not be used for monitoring patients.

Treatment Based on BPE Codes

  • Code 0: No treatment needed, but encouraging continued good home care is important.
  • Code 1: Treatment indicated, usually involving simple measures like plaque removal, scaling, and oral hygiene instruction.
  • Code 2: Treatment indicated, often requiring more complex services, such as scaling and root planing.
  • Code 3: Initial Phase Therapy, which may include scaling and root planing, oral hygiene instruction, and other treatments designed to address the underlying cause of the periodontal disease.
  • Code 4: Extensive treatment is required, potentially involving surgical procedures or other specialized care.

BPE Recording

  • BPE is recorded using a summary chart that indicates the highest code recorded for each sextant.
  • The codes are written in the order of the sextants: UR, UA, UL, LR, LA, LL.
  • For example:
    • 4 5 3* (UR, UA, UL)
    • 2* 4 4* (LR, LA, LL)
  • A “*” indicates a furcation involvement.
  • “-” indicates no teeth present in the sextant.
  • The BPE score represents a snapshot of the patient's periodontal status and can be used to guide further treatment.
