Study Notes
Basic Periodontal Examination (BPE)
- Aims to explain features, considerations, limitations, and usage of the BPE
- Explains WHO probe features and how they relate to its use.
- Demonstrates BPE index usage and describes clinical findings with needed treatments for each specific code
British Society of Periodontology Guidance 2019
- The BPE was initially developed by the British Society of Periodontology in 1986
- Previous versions were released in 1986, 1994, 2000, 2011, and 2016
- The next review is scheduled for 2024
- Stemmed form CPITN stands for Community Periodontal Index of Treatment Needs
- Also known as PSR and PEPE.
Periodontal Screening Tool Characteristics
- A simple and rapid screening tool that indicates the level of further examination needed
- Provides basic guidance on necessary treatments
- Not prescriptive, it sets a minimum standard of care for the initial periodontal assessment
- Should be used for screening purposes only, not for definitive diagnosis
Limitations of BPE
- Not suitable for monitoring patients
- Does not serve as a treatment outcome measure
- Does not provide a diagnosis
What a BPE Takes Into Account
- Calculus
- Bleeding
- Overhangs
- Pocket Depths
WHO/BPE Probe
- Considers ball height and features
- Determines rationale
- Includes the base and top of the 1st black band
- Requires consideration of the appropriate pressure
- Requires consideration of the method the probe is moved
Recording a BPE
- Divided into sextants: UR (17-14), UA (13-23), UL (24-27), LR (47-44), LA (43-33), LL (34-37)
- All teeth are included except 8's, unless the first and/or second molar is missing
- Has to contain a minimum of 2 teeth to be included as a sextant
- A sextant may have no teeth accountable
- An asterix indicates furcation
BPE Scoring Codes
- Code 0 has pockets <3.5mm, it includes no calculus or overhangs and no bleeding on probing, and the black band is entirely visible
- Code 0 requires no treatment and the patient should be encouraged to continue current practices
- Code 1 has pockets <3.5mm, includes no calculus or overhangs with bleeding on probing, and the black band is entirely visible
- Code 2 has pockets <3.5mm with supra or subgingival calculus/overhangs, and the black band is entirely visible
- Code 3 has a probing depth of 3.5-5.5mm and the black band is partially visible, indicating a pocket of 4-5mm which requires initial phase therapy
- Code 4 has a probing depth >5.5mm and the black band disappears, indicating a pocket of 6mm or more
- Furcation involvement also exists as a code
