AZBA PHYSIO CARE Employment Agreement
- The agreement is for employment at AZBA PHYSIO CARE, Head Office located at Gulab Devi Hospital, Jalandhar, Punjab.
- The applicant must provide personal details, including name, father's/husband's name, and residential address.
- The applicant must provide bank details, including bank name, account holder's name, account number, and IFSC code.
Terms and Conditions
- The agreement is valid for a minimum of nine months, and the selected candidate must complete the compulsory tenure.
- Failure to complete the tenure will result in no experience certificate being issued, and action may be taken against the candidate.
- One month's notice is mandatory before quitting the job, otherwise, one month's salary will be deducted.
- Working hours are eight hours a day, with working time subject to change according to the hospital schedule.
- The candidate is entitled to four days off in a month, with exceeding days being deducted accordingly.

- The candidate must follow the hospital dress code, which includes wearing a lab coat and ID card.
- The candidate is responsible for the department's equipment and the well-being of AZBA PHYSIO CARE.
Salary and Payment
- Salary will be transferred directly to the candidate's provided account.
- Salary will be transferred by the end of the month or by the second week of the next month.
Declaration
- The candidate must declare that they have read and understood the agreement and its contents without any hindrance or discrepancy.
