Study Notes

ASA Classification System Overview

  • Aims to assess and communicate a patient's pre-anesthesia medical co-morbidities.
  • Final Physical Status classification confirmed on the day of anesthesia care by the anesthesiologist after patient evaluation.

ASA I Classification

  • Represents a normal healthy patient.

ASA II Classification

  • Defined as a patient with mild systemic disease.
  • Includes patients with mild disease without significant functional limitations.
  • Examples include:
    • Normal pregnancy is classified as ASA II due to altered physical state.
    • Current smoker or social alcohol drinker.

ASA III Classification

  • Describes a patient with severe systemic disease but not a constant threat to life.
  • Examples of conditions classified as ASA III include:
    • Obesity (BMI ≥ 30) within three months of myocardial infarction (MI), cerebrovascular accident (CVA), transient ischemic attack (TIA), or coronary artery disease (CAD)/stents.
    • Pediatric patients with uncorrected stable congenital cardiac abnormalities, asthma exacerbations, poorly controlled epilepsy, insulin-dependent diabetes mellitus (DM), and morbid obesity.
    • Pediatric patients with malnutrition, severe obstructive sleep apnea (OSA), oncologic states, renal failure, muscular dystrophy, cystic fibrosis, a history of organ transplantation, brain or spinal cord malformations, symptomatic hydrocephalus, being born premature (PCA < 60 weeks), severe limitations from autism, metabolic diseases, difficult airways, long-term parenteral nutrition, infants under six weeks, and pregnant patients with severe preeclampsia, gestational diabetes with complications, or thrombophilic diseases needing anticoagulation.

ASA IV Classification

  • Represents patients with severe systemic disease that poses a constant threat to life.
  • Examples include:
    • Recent (< 3 months) history of MI, CVA, TIA, or CAD/stents.
    • Ongoing cardiac ischemia or severe valve dysfunction.
    • Severe reduction in ejection fraction (EF), shock, sepsis, disseminated intravascular coagulation (DIC), and acute or end-stage renal disease (ARD/ESRD) not undergoing regular dialysis.
