    Study Notes

    Cardiac Arrest Management

    • During cardiac arrest with an advanced airway, the recommended delivery rate of breaths is 10 breaths/min.
    • Electroencephalography (EEG) monitoring is not recommended for routine use after cardiac arrest.
    • Early delivery of epinephrine is encouraged in cardiac arrest when there is a delay in CPR or if the initial rhythm is asystole or pulseless electrical activity (PEA).
    • Cuffed endotracheal (ET) tubes are recommended for pediatric patients.
    • Routine use of cricoid pressure during intubation is not recommended.

    ACLS Algorithm Updates

    • The recommended atropine dose in the updated ACLS 2020 algorithm for adult bradycardia is 0.5 mg every 3-5 minutes, up to a maximum of 3 mg.
    • Epinephrine should be given every 3-5 minutes in nonshockable rhythms, including asystole and PEA.
    • The preferable route of medication delivery is IV or IO, as ET medication delivery is no longer recommended.
    • Multimodal neuroprognostication should be performed 72 hours or more after cardiac arrest to assess neurological prognosis.
    • For cardiac arrest in pregnancy, the recommended approach is to prioritize CPR, relieve suspicious aortocaval compression, and perform perimortem cesarean delivery if necessary.
