EHR522: Exercise for Metabolic and Renal Conditions Tutorial Four: Metabolic Syndrome Question 1: Define the diagnostic criteria for metabolic syndrome in Australia. Are there any compulsory conditions? five criteria ................................................................................................................................................................ - increased waist circumference - elevated fasting blood glucose (above 5.5mmol/L ) ................................................................................................................................................................ - elevated triglyverides - low HDL ................................................................................................................................................................ - high blood pressure 3/5 criteria need to be met and there are no compulsory conditions ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ Question 2: List some of the other terms by which Metabolic Syndrome has previously been known. ................................................................................................................................................................ Syndrome X Dyslipaedemuc hypertension ................................................................................................................................................................ The deadly quartet Insulin Resistance Syndrome ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ Question 3: Explain the physiological processes associated with insulin and glucose in an insulin-sensitive person ................................................................................................................................................................ In a healthy insulin sensitive person, glucose stimulates the release of insulin from pancreatic beta cell, which in turn reduces plasma glucose concentration through suppression of hepatic ................................................................................................................................................................ glycogenolysis and glucogenisis and simultaneous glucose uptake, utilisation and storage by the liver, muscle and adipose tissue ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ Question 4: Explain the physiological mechanism by which resistance training may facilitate a long-term reduction in body fat ................................................................................................................................................................ -increases muscle mass -increases resting metabolic rate ................................................................................................................................................................ - increases energy expenditure - decreases body fat ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ Question 5: Stuart, a 52 year old male, presents to you for an initial consultation following referral from his GP. Stuart was very active up until 5 years ago when he ruptured his right ACL and tore the posterior horn of his right medial meniscus during a touch football game. Stuart has trouble squatting beyond 45 degrees knee flexion and changing direction quickly, but he can run in a straight line without pain. The knee injury discouraged Stuart and he has been fairly sedentary for the past 5 years. Stuart works in a largely sedentary administrative role on a full-time basis. Stuart presents with the following data: LDLc: 4.5 mmol/L HDLc: 1.1 mmol/L Triglycerides: 1.9 mmol/L Blood pressure: 126/88 mmHg Fasting BGL: 6.2 mmol/L Body mass: 85kg Height: 1.77cm Waist circumference: 88cm a) Does Stuart have metabolic syndrome? Why or why not? b) Using the above information, explain to Stuart how an exercise program may be of benefit to him. c) Explain to Stuart how continuation of a sedentary lifestyle may be detrimental to him. d) Use the Exercise Program Development Table to prescribe a 6 – 8 weeks program for Stuart. HDL - 1.1 on the cusp but not bad Triglycerides - above 1.7 - so good but not great ................................................................................................................................................................ Fasting BGL - 5.5 is cut off so his is too high Blood pressure - is elevated even though it is just diastolic (only need systolic or diastolic to be too ................................................................................................................................................................ high) Waist circumference - 88 - cut off for men is over 94 so he is under which is good ................................................................................................................................................................ ................................................................................................................................................................ Exercise program may benefit Stuart through - improving BGL and insulin sensitivity ................................................................................................................................................................ - May reduce hypertension -Reduce likelihood of developing CVD ................................................................................................................................................................ -Further improve HDLc Reduction of body mass ................................................................................................................................................................ ................................................................................................................................................................ ................................................................................................................................................................ How continuation of sedentary lifestyle may be detrimental to him - significant increases in comorbidities in regards to obesity
