Proof of Income: Two (2) most recent pay stubs. Tax Returns, two (2) most recent years. (Business owners must include all schedules) Other: Proof of Insurance: Declarations Page/Insurance Binder Deductibles under $1000 (Comp & Coll) Vehicle information should be listed (VIN, Year, Make, Model) Driver should be listed on policy Include ACCU insurance lien information: Alliance Catholic Credit Union P.O. Box 924402 Ft. Worth, TX 76124 Title: Original “Green” Title (Boats are Blue) If lien exists: Release of lien Payoff from current lien holder including the following:  Financial Institution Name  Account #  10-Day Payoff Amount  Mailing address to send the payoff (this can be different than where payment is currently sent - check with Financial Institution) Purchase Agreement: Final Purchase Amount Salesperson name and contact info Include ACCU lien information: Alliance Catholic Credit Union 28800 Orchard Lake Rd Farmington Hills, MI 48334 Additional Documentation:
