Lecture 1 HLST2030 **What to focus on in your Reflections:** Main theme A clear understanding of what you learned: How the lecture material changed their perspective: Personal and clear concepts: Explain your ideas in 3-4 sentences and the evidence you put in it. 250- 350 sentences. [Problem with the healthcare system: Why?] The Canadian system - Connections are lost between people - communication is a problem. Sexism and racism are issues - people don't respect women as much or people of color and Indigenous people. - Inequity. Accessibility - people dont have family physicians or health insurance. There may not be enough doctors or nurses to help people. People may have the possibility to change their lifestyle, to help their life but people need access to get that good lifestyle - for example, to treat a disease early. To get diagnosed early. Need to prevent the illnesses. [Social and environmental factors:] What makes people sick? Health is determined by economic, social, and environmental conditions. They are all connected and they impact health. Social isolation - Taken away from their communities - ex: Indigenous people. Stress from everyday life. - such as climate issues, fires, instability in weather, and increased loss of finance. Processed food, unemployment, homelessness, and depression cycle. Everybody is affected - young, old, middle-aged, they are affected by mental pain such as suicide. - pressure, addiction, chronic diseases. Having a better health system - it can change our social, environmental, and economic factors. Need food high in nutrition for people who are unhealthy, and that have been on medication and processed food. People need a proper place to live. People in a bad place can get raped, shot, or injured, this needs to be taken into consideration. [Over-medicalization of society: ] It is driven by fake profits. A drug for weight that has long-term effects and can become an addiction. It\'s a trap for money. There is demand in health care for profits and people pay the consequences. A motto: A pill for everything. They do pills for everything and people need to change their mindset. [Lack of Accessibility:] Barely any family physicians, and this affects people and causes expensive treatment. Crowdedness of emergency rooms and no room for people. Over 12 hours of wait time in an emergency room. Why is it called an emergency room? It lost its name. It\'s not an emergency anymore. It\'s one thing acknowledging it and another thing, doing something about it. Must know the complexity and manage it. [What perspective must be used to address the problem?:] **How we perceive the issue impacts how we respond.** If we have the wrong perspective, we won\'t solve the issue. A lens that is based on a framework that allows you to look into something broader. People must look into what is happening close in society. How does something end up in this situation? Knowing that we can help get out of that situation. Need the diversity of perspectives. People need different perspectives. It affects your position from where you are viewing. If you are the patient, the doctor, the government, etc. **The position from where you are viewing impacts your perspectives.** If you are the patient, the doctor, the government, etc. How we perceive the position, affects our perspective. **When we are surrounded by people that share the same thoughts then they all start to think that is reality.** Work as teams - pharmacists, doctors, nurses, etc. There are issues everywhere. People need to function and work together. "If I were given one hour to save the world, I would spend 59 minutes defining the problem and 1 minute solving it". - Albert Einstien. **This is the importance of understanding a problem, then it is easy to solve it.** People have spent a long time trying to solve a problem without knowing what the problem is. That is what people in healthcare do, they do things that dont solve the actual problem. [How to solve a problem?: ] An elephant and 5 blindfolded. 5 men are trying to understand what it is they are touching. They do not know the whole thing. - this symbolizes the position of the system and shows that the information is limited and can not solve the problem without understanding the system. People must understand the system. **Reductionsilms:** are dividing the system into its parts to understand its individual properties. Ex: the human body. People must break down the system to understand it. The parts are reflective of the system. **Holism:** systems are viewed as a whole so each component is seen in its context and to one another. Ex: The Brain - people cannot take it apart and get the same brain. The connections are lost. The parts do not reflect the system. Parts of the brain are not part of the brain. It functions in relation to the parts of the brain. Always check your assumptions. People must look beyond the details and see the whole picture. People must think about holism. Understanding the system as a whole and the fundamental principles that guide it is key. The principle is something that is given. Pouring resources into health care is not the solution, it is not the principle of the solution. Must involve some type of communication, and find the principle. Question yourself and find out what are the principles given. **Solving complex problems doesn't need complex solutions.** Ex: a needle out of a haystack, it just needs a metal that gets metal out. **There should be innovative ways to solve problems.** People need to shit thier mindset and get a new one to understand systems. Interrelationships: how do things relate? needing proper relations and perspectives that people are affected by. Perspectives: people have different thoughts of different things. Boundaries: can limit communications and how they affect people and options. When people intervene, they must think about how it affects the whole system. Cost, Quality, and Access affect each other. Changing one influences and changes the other two. [The second challenge in Healthcare:] The a need to address the interdependence of health as it relates to the social and economic condition. Expanding the boundaries of Cost, Quality, and Access. It is the community that gets affected and it is the community that affects health. - safe zones, price markets. **Social determinants of health:** Childhood, housing, education, social support, family income, employment, communities, and access to health services. To address these conditions, people need to talk about healthy communities and changing the policies that keep people trapped, unfunded, and unproductive properties. It is with the people you interact with and the policies that need to change. Health needs to be redefined. It is not just the absence of diseases, but the ever-presence of well-being. People need to address the social determinants of health to address the healthcare system. **Change the conditions, this is the solution.** People need to move away from sidel thinking. People think about the output of the system, not the input part. People need to think in the system. [What management perspectives do we pursue in improving the system? ] **Management perspectives:** Classical management school - Social network theory - Human relations school - These are how to apply the problem with the right perspective.
