Review: Reverse Total Shoulder Arthroplasty Reverse shoulder arthroplasty (RSA) is a type of shoulder replacement surgery where the normal ball-and-socket structure of the shoulder joint is reversed. In a typical shoulder joint, the humeral head (ball) fits into the glenoid cavity (socket) of the scapula. However, in RSA, the positions are reversed: a metal ball is attached to the glenoid, and a socket is created at the top of the humerus. This reversal alters the mechanics of the shoulder to better utilize the deltoid muscle for lifting the arm, which is particularly beneficial for patients with rotator cuff tears that cannot be repaired. Mechanics: RSA changes the biomechanics to rely on the deltoid muscle, while ASA preserves the natural shoulder mechanics. Indications: RSA is often used for patients with irreparable rotator cuff tears or severe cuff tear arthropathy, and fractures. This may result in significant cost saving opportunities as a result of lower operating room time, lower intra- and post-operative complications, and revisions. This allows a surgeon to optimize construct selection intraoperatively without having to worry about bringing in backup instrumentation potentially improving intra-operative efficiency.
