BOCA RATON POLICE SERVICES DEPARTMENT Standard Operating Procedure 41.08 DEATH INVESTIGATION FORM Revised: new I. PURPOSE: The purpose of this standard operating procedure is to instruct law enforcement officers (LEOs) on how to properly complete the Palm Beach County Medical Examiner’s Death Investigation Form. II. PROCEDURE: A. DEATH INVESTIGATION FORM REGISTRATION PROCEDURES: 1. LEOs are required to register with the Palm Beach County Medical Examiner’s Office online at: http://discover.pbcgov.org/medicalexaminer/Pages/default.aspx 2. To register click on the “Death Investigation Form” link in the upper right corner, which displays the “Registration/Login Page.” 3. Click the “Register” box and input the personal information required on the form. 4. Once a LEO registers, it can take up to 24 hours before he/she is accepted as a user, and he/she will receive a confirmation email when he/she can utilize the system. 5. login. COMPLETING THE DEATH INVESTIGATION FORM: 1. The LEO conducting the death investigation shall complete the Death Investigation Form as thoroughly and accurately as possibly. 2. The LEO shall access the Medical Examiner’s link to complete the form at: http://discover.pbcgov.org/medicalexaminer/Pages/default.aspx Effective: September 18, 2019 Revised: Death Investigation Form SOP No. 41.08 Page 1 of 3 3. Prior to submitting the form to the Medical Examiner’s Office, the LEO shall complete all nine of the following sections: a. Law Enforcement Information b. Decedent’s Information c. Next of Kin d. Discoverer of Body e. Death Information f. Body Released to g. Physician’s Information h. Medical History i. Narrative 4. The narrative section of the form should be brief and describe what occurred just prior to, during and/or just after death. 5. Once the form is completed the LEO may review it to ensure that the information on it is correct. 6. The LEO shall then send the finalized form to the Medical Examiner’s Office by clicking the “Submit Form” box on the top right corner of the form. 7. Once the Death Investigation Form is submitted to the Medical Examiner’s Office, the LEO completing the form will be unable to modify the information on the form. 8. The LEO shall contact the Medical Examiner’s Office directly at (561) 355-4357 if he/she needs to make any changes to the form once it is submitted. 9. Once the document is submitted, the submitting LEO will be given the option to see the completed form by clicking “View PDF.” 10. On the main page of the Death Investigation Report, in the top right corner of the page, LEOs will be able to print a blank copy, a draft copy or a finalized form. 11. The LEO shall print a copy of the final form by clicking on “View My Finalized Forms.”, then click on “View,” and view, print or download the form as needed. 12. The LEO shall initial next to their name on the printed form and submit it his/her immediate supervisor. 13. The submitting LEO’s supervisor shall review the form, initial next to their name if the form is accurate and complete, then forward the form to the Records Section.
