HOFFMAN ESTATES FIRE DEPARTMENT STANDARD OPERATING GUIDELINES SOG Category & Identification Number: EFFECTIVE DATE: ADMINISTRATIVE – 014 January 1, 2022 NIMS COMPLIANT SOG Title: Accident/Injury Reporting Revision: 4 APPROVED BY: NUMBER OF PAGES: Re-evaluation Date: Alan Wax January 1, 2024 Fire Chief 3 PURPOSE The purpose of this policy is to establish a system of reporting all vehicle accidents and establish guidelines for medical treatment for work-related injuries. ACCIDENT REPORT REQUIREMENTS: All Fire Department personnel will notify the Hoffman Estates Police Department when involved in a vehicular accident. POLICY A. Shift Commander: 1. Have the driver supply the information for completion of a Hoffman Estates Police Department accident report. 2. Have the driver complete, in duplicate, a State of Illinois Division of Highway Report of Motor Vehicle Accident. In all cases of accidents in which Village-owned vehicles are involved. When any of his/her personnel are struck by a motor vehicle or other motorized equipment while on duty. In all cases involving any damage to any Village property in excess of $100.00 by a motor vehicle and the driver of the vehicle can be identified. 3. Will ensure that the Company Officer will complete Village of Hoffman Estates Supervisor's Accident Investigation and the driver completes the Hoffman Estates Fire Department Employee Accident/Injury Report. 1 4. Shall report, at once, to the Deputy Fire Chief any motor vehicle accident that occurs to a Department vehicle. After notifying the Deputy Fire Chief, the Chief’s Notification Button on the Shift Commander’s phone should be utilized to notify the remaining Administrative Chief Officer’s. 5. Shall review all related reports for accuracy, consistency, legality and completeness. 6. Shall submit a written report to the Deputy Fire Chief as to the facts of the accident no later than the end of the duty day. B. The investigating police officer shall obtain all the information pertaining to the accident from the other driver, witness, pedestrian, etc., and fill out the required reports. WORK RELATED INJURIES When an employee is injured while on duty, medical attention is frequently necessary. This guideline has been designed to ensure expeditious and quality medical treatment of any injured employee and to provide reasonable cost controls. PROCEDURES A. When an accident occurs resulting in an employee injury, it will be necessary to determine if medical attention is needed, and if so, whether the injury is serious enough to transport the injured employee to a medical facility. B. The injured employee, fellow employees, or supervisor should never take a chance when dealing with somebody's health. If they are not sure how serious the injury is, the injured employee should go to the St. Alexius Medical Center as soon as possible. Life-threatening injuries include, but are not limited to, crushing chest pains, seizures, head injuries, severe respiratory problems, loss of consciousness, hemorrhaging, open fractures and severe burns and should be directed to the hospital. When an on-duty accident occurs which results in life-threatening injuries, the Paramedics should be contacted for immediate care and transport. In such an instance, the first employee on the scene should call for Paramedic attention. C. If the injury is not life threatening, the employee is encouraged to go to Physicians Immediate Care, located at 405 Golf Road, Schaumburg. The clinic is open Monday through Friday, 8:00 a.m. to 8:00 p.m. Saturday and Sunday 8:00 a.m. To 4:00 p.m. The specific clinic used may change from time to time, depending on service, hours of operation, and cost. 2 Common injuries which may be brought to Physicians Immediate Care include, but are not limited to, sprains, simple closed fractures, lacerations, cuts, bruises, minor burns, and insect and animal bites. D. If the injured employee will be going to Physicians Immediate Care, or any other health care provider, the following guidelines should be followed: 1. Physicians Immediate Care will provide all employees with a Duty Status Report, indicating when they may resume their normal work duties or be assigned to modified duty. If the employee chooses another health care provider, the employee will provide a blank Duty Status Report to be completed by the health care provider. 2. The injured employee will not be required to make payment at the time of treatment, but rather the Village will be billed for services rendered. Remember, regardless of the type of injury, all forms must be completed and proper notification given to supervisors within prescribed times. E. As soon as possible after an employee injury has occurred, the Shift Commander shall report the injury to the Deputy Fire Chief and the Village Risk Manager. After notifying the Deputy Fire Chief and the Village Risk Manager, the Chief’s Notification Button on the Shift Commander’s phone should be utilized to notify the remaining Administrative Chief Officer’s. F. Record of Personal Injury. The following forms are to be completed: o Supervisor's Accident Investigation Report o Employers First Report of Injury or Illness (Form 45) o Employee Accident/Injury Report All injuries shall be reported and an Employee Accident/Injury Report completed by the department member who was injured, co-signed by that person's immediate supervisor or Company Officer and the Shift Commander. The injured employee’s supervisor shall complete the Employers First Report of Injury or Illness (Form 45) and a Supervisor's Accident Investigation Report. All reports will be completed and forwarded through channels to the Battalion Chief of Training and Safety at the end of their tour of duty. EXCEPTIONS REFERENCES DEFINITIONS End of Document 3
