Clinical Lab Test Notes: 1.​ If the patient has a temperature of 102°F: ○​ Symptoms: High fever, elevated heart rate 2.​ Obtaining oxygen saturation for a client: ○​ Choose the finger with capillary refill less than 2 seconds. 3.​ If a patient has a temperature of 35°C (hypothermia): ○​ Intervention: Remove excess clothing, apply a cold compress, and remove excess blankets. 4.​ If a patient shows orthostatic hypotension: ○​ Check blood pressure supine; a decrease of 20 mmHg systolic indicates orthostatic hypotension. 5.​ If a patient has oxygen saturation of 88% (O₂ %): ○​ Intervention: Encourage a deep breath. 6.​ Palpating radial pulses (right: 80 bpm, left: 48 bpm): ○​ Report as 4+80. 7.​ Patient with constipation and recent bowel movement: ○​ Symptom: Decreased heart rate. 8.​ If a patient has an irregular heart rate: ○​ Intervention: Auscultate the apical pulse. 9.​ Patient’s heart rate is 56 bpm: ○​ Expected symptom: Dizziness (bradycardia). 10.​Patient with dehydration and hypotension: ○​ Intervention: Increase fluid intake. 11.​Patient with postural hypotension: ○​ Assist them from lying down to a standing position; systolic pressure may be higher. 12.​If a patient has an elevated temperature: ○​ Elevated temp of 108°F causes an increased heart rate. 13.​Obtaining electronic blood pressure: ○​ Elevate the client’s arm above the level of the heart (brachial artery).
