Example: New member: could be referred by "x" or received mail piece, or saw location while driving. 2\. Explanation of time on job if less than 2 years. a\. Example: Change companies for better pay b\. Example: Graduated college and beginning career in field of study 3\. If there are multiple applications for the same member, reference them. a\. Example: Member is also applying to refinance auto from Chase along with this credit card application. 4\. State in the notes if proof of income is scanned in and verified (such as last two recent pay stubs), or if borrower receives direct deposit with ACCU. Self-employed borrowers will need last two years of full personal tax returns. 5\. If this is a signature loan request, what is the purpose of funds? a\. Example: Member needs funds for repair to furnace. b\. Example: Planning vacation this summer and needs funds to help pay expenses. 6\. If the purpose of funds is for debt consolidation, list the debts to be paid. Does borrower plan on closing these trade lines or keeping them open? a\. Example: Chase \$5,000 close, Discover \$3,500 leave open, Bank of America \$7,200 close 7\. If the credit report reflects a delinquent trade line 30 days or greater in the past 6 months, give an explanation in the notes. a\. Example: Member was 60 days past due three months ago on mortgage due to a seasonal job layoff. 8\. Bankruptcy in the past two years -- provide explanation as to why they filed. a\. Example: Member recently went through a divorce and household income was decreased significantly. 9\. If the credit report has a fraud alert - verify the member's identity, and state in the notes how this was verified. a\. Example: Contact member at phone number listed with fraud alert and asked verifying questions.
