StarCare Lubbock, Tx Mobile Crisis Outreach Team ● StarCare’s highly trained and dedicated Mobile Crisis Outreach Team (MCOT) provides intervention and assessment for appropriate interventions and resolutions 24/7 to people suffering from a mental health crisis. ● Like any other health crisis, it’s important to address a mental health emergency quickly and effectively. With mental health conditions, crises can be difficult to predict because, often, there are no warning signs. Mental health crises can occur even when treatment plans have been followed and mental health professionals are involved. Unfortunately, unpredictability is the nature of mental illness. ● 24/7 Crisis Helpline: 806-740-1414 or 800-687-7581 Extended Observation Unit ● StarCare’s Extended Observation Unit, located inside the Sunrise Canyon Complex, is a 23-hour observation unit where people who are experiencing a mental health crisis can be admitted in order to stabilize the crisis in the least restrictive environment. ● In most instances, the emergency room is not conducive for a person in crisis – there they’re told to wait and like any health crisis it’s important to address a mental health crisis quickly and effectively. Waiting in crisis can lead to harming oneself or others or damaging property and can end with a call to law enforcement. Jail, like an ER, is also not the most appropriate place for a person in crisis. ● In the Extended Observation Unit a person experiencing a crisis: ○ Has access to see a psychiatrist upon admission ○ Is observed by a psychiatric registered nurse ○ Is discharged at the end of 23 hours, if stable, and is ■ provided with a 10-day supply of medication(s) ■ provided with a referral for a follow-up appointment as appropriate ○ Is referred to an inpatient psychiatric facility if appropriate at the end of 23 hours Intellectual Disabilities Services StarCare provides and coordinates services for people with Intellectual Disabilities. Services include: ● Home and Community Based ● Texas Home Living ● Intermediate Care Facilities ● Day Habilitation. Day Habilitation Services StarCare’s day program is open 8 a.m.-2:30 p.m. Monday through Friday. In this program, our staff organize and provide supervision for on- and off-site day activities to provide opportunities for socialization and community interaction. Activities include: ● Enhancement of daily living skills ● Participation in community events ● Rights and self-advocacy training ● Employment assistance and job coaching Personal Assistance Services/Habilitation and Supported Home Living/ Community Support Transportation ● ● These services are provided by StarCare’s partners in a person’s home and community on an hourly basis. Support services range from cleaning, cooking, meal preparation, and shopping to community activities such as eating out, movies, and other recreational activities. Transportation is also provided as a part of these services. These services are provided based on need. To Qualify for Personal Assistance Services/Habilitation and Supported Home Living/Community Support Transportation People Must: ● Live in their own home or with family ● Be enrolled in one of the following ● Home and Community-Based program ● Texas Home Living Program ● General Revenue program Residential Services StarCare’s group homes provide 24-hour services that include: ● Supervision for safety and security ● Medical appointment assistance ● Reminders or assistance to complete daily living skills ● Meal planning and preparation ● Transportation ● Shopping ● And more Up to four people can live in a Home and Community-Based program home. Up to six can live in Intermediate Care Facility homes. Most people living in these settings attend a day program Monday-Friday. Mental Health First Aid Training: SOMETIMES FIRST AID ISN’T A BAND AID – IT’S YOU! ● ● ● ● StarCare provides in-person and virtual Mental Health First Aid (MHFA) Training to groups and individuals who reside in Lubbock, Lynn, Cochran, Crosby, and Hockley counties. The course is designed to give members of the public key skills to respond in a mental health emergency and offer support to someone who appears to be in emotional distress. Most people know how to recognize and appropriately react to medical emergencies, but there is little knowledge in the general public about what to do in a mental health crisis. First Aiders will complete a 2-hour self-paced class and then participate in a 4.5- to 5.5-hour instructor-led class using videoconferencing technology. People interested in MHFA can attend one or two separate courses (adult and/or youth) that introduce participants to: ○ Unique risk factors and warning signs of mental health problems ○ The importance of early intervention ○ Helping a person in crisis or experiencing a mental health or substance use challenge The virtual training includes content on trauma, addiction, and self-care and is genderneutral and culturally relevant. In addition, the Youth MHFA training also has new information for adults working with elementary-age children, including the topic of social Mental Health First Aid Training ● ● ● Just as CPR training helps a person with no clinical training assist an individual following a heart attack, MHFA training helps a person assist someone experiencing a mental health or substance use challenge or a mental health crisis, such as contemplating suicide. Through experiential activities and role-playing scenarios, participants will: ○ Learn how to assess a mental health crisis ○ Learn about the risk factors and warning signs for mental health or substance use problems ○ Build an understanding of the impact of illness on individuals and families ○ Learn information about evidence-supported treatment and self-help strategies ○ Learn how to provide initial help and connect people to professional, peer, social, and self-help care The goal of Mental Health First Aid is to increase mental health literacy, not to learn to diagnose or to provide therapy or counseling. Treatment is designed to reduce symptoms of mental illness and is provided in a secure environment where care is provided by clinical staff and supervised by licensed psychiatrists. In accordance with individual treatment plans developed to be appropriate to the needs and interests of the patient and directed toward restoring and maintaining each person’s optimal level of physical and psychological functioning, Sunrise Canyon Hospital offers the following: ● Medication management ● Mental health education ● Therapeutic activities ● Social Services People in need of hospital access are admitted through one of the following ways: ● Mental health crisis ● Voluntary admission ● Emergency detention center ● Magistrate’s warrant Youth Empowerment Services: Assisting Children and Adolescents with Serious Emotional Disturbances When a child or adolescent is diagnosed with a serious emotional disturbance, the reality of the need for additional support and home modifications for the youth’s care and respite for the youth’s caregivers can be overwhelming. Youth Empowerment Services (YES) provides assistance to children and adolescents with serious emotional disturbances so that they can live in the community with their families. YES Goals ● Reduce out-of-home placements and inpatient psychiatric treatment ● Provide a more complete continuum of community-based services and supports ● Ensure families have access to parent partners and other flexible non-traditional support services ● Prevent entry into the foster care system and relinquishment of parental custody ● Improve children and adolescent clinical and functional outcomes YES Services ● Adaptive Aids and Supports ● Community Living Supports (CLS) ● Family Supports ● Minor Home Modifications ● Non-Medical Transportation ● Paraprofessional Services ● Respite ● Specialized Therapies ● Supportive Family Based Alternatives (SFA) ● Transitional Service To Qualify for Youth Empowerment Services The Child or Adolescent Must: ● Be 3-18 years old ● Live in Lubbock, Lynn, Cochran, Crosby or Hockley counties ● Reside in a non-institutional setting with the child’s or adolescent’s legally authorized representative; or in the child’s or adolescent’s own home or apartment, if legally emancipated ● Have serious functional impairment or acute severe psychiatric symptoms as indicated by: ● Risk of Self-harm ● Disruptive or Aggressive Behavior ● Lack of Family Resources ● School Behavior ● Current Psychiatric Diagnosis ● Have a reasonable expectation that, without YES, the child or adolescent would qualify for inpatient care ● Be Medicaid eligible Rehabilitation StarCare Specialty Health System’s Rehabilitation services are generally intended for individuals who require intensive rehabilitation to increase community connection, establish support networks, and develop coping strategies in order to function effectively in their social environment. The general focus of intensive rehabilitation services is to support the individual through a team approach that engages him/her as a key partner in their rehab. The goal is to stabilize symptoms, improve social function, develop self-advocacy skills, increase natural supports in the community, and/or sustain improvements made in more intensive services. The emphasis is on bolstering identified strengths and reducing functional needs through: ● medication management ● skill-training activities ● symptom management ● independent living ● self-reliance ● substance abuse education ● employment strategies ● impulse control ● effective interaction with peers, family, and community. To be eligible to receive Rehabilitation services: Adults must: ● Be 18 years or older ● Have a diagnosis of Schizophrenia, Bipolar Disorder, or Major Depressive Disorder, each with psychotic features ● Have high needs in family functioning, natural supports, employment, residential stability, substance use, and criminal behavior ● Have had multiple psychiatric crises Youth must: ● Be 3 to 17 years old ● Have a mental health diagnosis ● Be at risk of having to move away from their home or school ● Be enrolled in special education ACT FOR SPECIALIZED MENTAL HEALTH TREATMENT The Assertive Community Treatment (ACT) Team, made of qualified mental health professionals, is our most intensive service provision team to provide specialized mental health treatment that supports an individual’s ability to remain in the community by identifying and addressing their personal recovery goals. A person receiving ACT services will see their psychiatric provider for: ● In-home medication management, monitoring, and administration weekly and/or daily as determined by the psychiatric provider ● Evidenced-based skills training and education with use of the Illness and Recovery Management Curriculum To be eligible to receive ACT Team services, people must: ● Be 18 years or older ● Have a diagnosis of Schizophrenia, Bipolar Disorder, or Major Depressive Disorder, each with psychotic features ● Have recent psychiatric hospitalizations TARGETED CASE MANAGEMENT StarCare Specialty Health System’s Targeted Case Management services support families to make positive changes that help people recover from mental illness and foster resilience. In Targeted Case Management Services, individuals demonstrate a low to moderate level of need, and an intensity of services and resources are focused on one primary need. In this service, individuals receive either counseling or skills training at the core of the treatment. Services are based on the individual’s strengths, needs, and preferences. Benefits include: ● Reduction of mental health symptoms ● Development of new communication skills ● Development of new ways to manage emotions ● Development of new ways to relax and create ● Improved social functioning at home and in the community ● Services that fit into the individual’s schedule ● Services provided where the individual lives To be Eligible to Receive Targeted Case Management Services: Adults must: ● Be 18 years or older ● Have a diagnosis of Schizophrenia, Bipolar Disorder, or Major Depressive Disorder, each with psychotic features ● Have high needs in family functioning, natural supports, employment, residential stability, substance use, and criminal behavior ● Have had multiple psychiatric crises Youth must: ● Be 3 to 17 years old ● Have a mental health diagnosis ● Be at risk of having to move away from their home or school ● Be enrolled in special education Substance Use Services Support: StarCare is Your Partner in Recovery StarCare provides services to those who are looking for and in need of substance use services and/or support through Outreach, Screening, Assessment, and Referral (OSAR). SCREENING AND ASSESSMENTS ● StarCare’s dedicated, licensed staff provide outreach, screenings, assessments, and referrals to those who are actively seeking substance use services and/or support. The OSAR team conducts verbal screenings to gather information pertaining to the severity of use and how use has impacted day-to-day functioning or mental health in order to provide appropriate referrals to recovery resources Interventions Following the screening and assessment, individuals can expect short-term case management services in addition to the opportunity of up to six brief motivational interviewing sessions with their OSAR counselor. Support OSAR counselors are available for additional support and case management until the individual successfully enters into their referred treatment service. Our OSAR counselors also remain as a point of contact for future recovery support needs. For uninsured or underinsured individuals, the OSAR Team can determine if you are approved for full or partial financial assistance toward your referred treatment services. StarParent Assistance with Prenatal and Postpartum Learning and Education Intervention Services for Expecting and Parenting Individuals with Substance Use Disorder (SUD) or Who Are at Risk of Developing a SUD (CPS)StarCare’s StarParent Program coordinates with community agencies to help pregnant and postpartum women obtain the resources needed for mothers and their children at no cost. The program also provides community-based, gender-specific intervention services, at no cost, to parenting males and expecting fathers who are at risk for involvement, or who are currently involved, with the Department of Family and Protective Services (DFPS) who have a substance use disorder (SUD) or who are at risk of developing a SUD. Services Provided ● Evidence-based parenting and substance use education to schools, community agencies, and individuals that includes: ● Reproductive health ● Fetal & child development ● HIV and STDs ● Nurturing parenting values ● Coordination of services with other community agencies to promote health and well-being of the individual and family ● Substance use screening and assessment to identify issues related to alcohol and drugs ● Linkage to community resources such as housing, transportation, daycare, food, clothing and many other necessities ● Referrals to cooperating agencies for help with prenatal-postpartum needs, counseling, employment, clothing, and additional needs ● Alternative activities – including picnics, games, arts and crafts, and trips to museums and libraries – focused on helping families address communication, family roles, and stress management, while supporting family relationships To Qualify for Parenting Awareness & Drug Risk Education: Individuals Must: ●[Women] Be pregnant or postpartum (includes the period from birth through 18 months of the infant) ●[Males] Be referred by the DFPS or at risk for DFPS involvement who are identified as being at risk of having, or who have, a SUD and who have children under the age of six. ●Have SUD risk factors including: a.Teen pregnancy (including females through age 19); b.Current or past involvement with DFPS or other child protective services (CPS) agencies; c.Current or past use/abuse of, or dependence on, alcohol and/or drugs; d.Current or past domestic violence; e.Current or past mental health problems, especially perinatal mood disorders; f. Living in a household with a person who abuses alcohol or drugs; g.Current or past history of sexual, emotional, or physical abuse; h.Insufficient, late (third trimester), or no prenatal care; or i. Financial distress Medication Assisted Treatment StarCare’s Medication Assisted Treatment (MAT) program, formerly known as the Methadone Clinic, provides treatment for adults to alleviate the effects of opioid withdrawal. The program works to meet individualized needs for treatment, including one-on-one and group counseling. To Qualify for Medication Assisted Treatment Individuals (Including Pregnant Females) Must: ● Be a Texas resident ● Meet the criteria for a moderate to severe opioid use disorder Substance Use Disorder Outpatient Program StarCare’s SUD Outpatient Program provides services to those who struggle with substance use issues and who do not require a more structured environment such as residential services to meet their treatment goals. While in services, clients will attend weekly individual counseling and group counseling sessions. During their time in services, the client will work with a counselor to address identified problem areas by teaching healthy coping skills to strengthen their ability to maintain their sobriety and care for their mental and emotional well-being. Program Goals ● Program staff will assist the client in identifying and addressing problems, which create challenging situations for maintaining recovery. The program will also emphasize relapse prevention skills for clients to develop a drug-free lifestyle to improve their overall well-being. Through group counseling, clients will also learn about the relationship between trauma and substance use through the Seeking Safety Workbook. ● Program staff will also assist clients in developing necessary life skills such as problem-solving skills, maintaining steady employment to become economically self-sufficient, self-care, enhancing relationship skills to promote family and social rehabilitation, and increasing skills for drug-free living. Clients will be provided education on tobacco cessation, overdose prevention, and HIV/AIDS and STDs. Program Requirements ● All persons presenting for services must first be screened by an OSAR representative and referred to the SUD Outpatient program. To schedule a screening, the individual may contact 806-740-1421. Clients are also able to contact Justine Vaughn for instructions for entering the program at 806-407-2308. ● Individuals who are presenting for services must be 18 years or older, and have a substance use disorder. ● Individuals presenting for services must provide proof of residency, income, and identification. If they are unable to do so, they must sign an attestation letter. ● Individuals presenting for services must be a resident of Texas. The SUD Outpatient Program will serve individuals within the following counties: Lubbock, Crosby, Lynn, Hockley, and Cochran. Program Curriculum The SUD Outpatient Program utilizes the following curricula for group counseling sessions: ● Seeking Safety: A treatment manual for PTSD and substance abuse ● Hazelden’s IDDT Recovery Life Skills Program ● SAMHSA’s Matrix Model: Relapse prevention
