Hyponatremia Cause: low sodium consumption, vomiting, diarrhea, diuretics "thiazides”, Addison disease, High ADH; Fluid overload from hypotonic solution, CHF, renal failure, diabetes insipidus SIGNS: seizures, abdominal cramping, lethargic, tendon reflexes diminished, trouble concentrating, loss of urine and appetite, orthostatic hypotension (FALL RISK), overactive bowel sounds, shallow respirations, muscle spasms, tachycardia (weak/thready) Interventions Hypertonic IV solution if hypovolemic Restrict fluids and give diuretics and dialysis if hypervolemic If due to SIADH,then administer ADH antagonist (declomyocin; do not give with food) Instruct patient to consume sodium rich foods
