Self-Direction Allows greater access to services in rural areas. Self-Direction Expands labor force by creating new opportunities for caregiver employment. Self-Direction Honors cultural uniqueness of individuals with workers. Self-Direction Allows participants to be served by individuals they know and trust. Continuity is critical particularly for those with dementia. Self-Direction Preferences may change over time. Self-Direction Cost is same or less that traditional personal care services. Self-Direction Facilitates easier access to services on nights and weekends. Self-Direction Flexible budgets allow for persons to have greater access to community activities. Participant Directed Goods and Services (PDGS) can include: Dental Vision Health-gym memberships, swim membership, home exercise equipment Sensory items Adaptive equipment Therapy Services Music, Dance Art Hippotherapy Aqua therapy Weighted blankets Music lessons Easy Grip Key Turner Lever Doorknob Handle Extender. Employee obligations such as Federal, state and unemployment withholdings, unemployment insurance and payments to Social Security and Medicare payments are deducted and reported by the FMS on the behalf of the employer of record. The FMS provider serves as an agent to the participant to ensure compliance, filings, and payments for Federal, State and other local employee obligations are made accurately and timely.​ This option provides the most power, choice and control over services.​ This option is most popular.​ Agency with Choice Option An agency such as a home health agency or other provider agrees to serve as the employer for tax purposes.  The participant oversees recruiting, hiring, and managing the worker. ​ Many Intellectual and Developmental Disability programs use this model.​ Scrutiny must ensure the participant’s rights are fully protected. ​ Employer Authority means that a member can directly hire attendants of his/her choice and train, manage, schedule, and dismiss attendants as needed.​ Employer Authority is most common form of employer authority is to hire an attendant to assist with activities of daily living (e.g., bathing, dressing, grooming, etc.) or instrumental activities of daily living (e.g., meal preparation, running errands, shopping, etc.)​ Employer Authority means hired individuals are considered domestic workers and all national, state, and local labor laws apply to them.
