Engagement of UP & P in SDI, CM, EPB, and Settlement initiatives. Build FDP focusing on 7 Domains. Move annual household income above $35,000 factoring in household size and cost of living in various cities. Prioritize UP & P families followed by AtRisk families. 4 Poverty Elimination System: Overview Holistic Coordinated Case Management Services for Poverty and SSN cases through the Access Platform Case Management System Poverty (VCM), SSN, Regional SSN FMs Settlement Settlement Case Managers/ Coordinators Settlement for New Immigrants and holistic service delivery Skills Developme nt Initiative (SDI) Stigma-Free Platform designed to move people out of income poverty. Primary focus is continuing education Quality of Life Volunteers (QLVs) Aga Khan Economic Planning Board Economic (AKEPB) Services (ES) Volunteers Jobline – Direct or through ACCESS as well as Small Business Initiative WHAT IS THE FAMILY MENTORSHIP APPROACH? Family Mentorship Approach is a dynamic process of engagement through which families set and reach their own goals. Family to be the holders of their destiny. Family develops self reliance.
