Common Medications for antibiotics are: Amoxicillin Bismuth Clarithromycin Metronidazole – w/ food for GI distress Tetracycline Tinidazole Antibiotic therapy includes a combo of 2-3 antibiotics for 14 days to increase effectiveness and minimize resistance to medication. Take the full course of medicine. Adverse Effects: nausea and diarrhea Histamine2 – receptor antagonist block H2 receptors that suppress secretion of gastric acid and lower the concentration of hydrogen ions in the stomach. Cimetidine Famotidine Nizatidine -PO Only Complications of Histamine2-receptor antagonist are blocked androgen receptors which cause: Decreased libido Gynecomastia Impotence (Luckily these side effects go away when the medication is stopped.) -Side effects that affect the CNS are: lethargy, hallucinations, confusion, restlessness (more common in older people with kidney or liver disease) -Side effects that affect the GI are: Constipation, diarrhea, nausea can also occur and should be reported to the dr. CAUTION: PREGNANCY/LACTATION HIGH RISK PNEUMONIA – Decrease gastric acid leading to bacteria growth in lungs and stomach COPD KIDNEY IMPAIRMENT- reduce dosage Interactions: -Cimetidine can inhibit med metabolizing enzymes and increase the levels of warfarin, phenytoin, theophylline, and lidocaine. Monitor blood levels such as PT and INR. -Antiacids cand decrease absorption of H2Ras so DO NOT take antacid 1 hr before of after H2RA. Smoking can decrease the effectiveness of H2RA. avoid after the last dose of the day. Delays healing. Nursing Administration -Famotidine can be administered IV for acute situations -Treatment of peptic ulcer disease is usually started as an oral dose 2x a day until ulcer is healed, -followed w maintenance dose, taken at bedtime. -Tell dr. about GI Bleeding (coffee ground emesis). Avoid alcohol, and limit NSAIDs. -Increase fiber and fluid for constipation prevention. Adhere to medicine regimen.
