Ovaries are enlarged secondary to multiple cyst formations within the ovaries. Causative factors seem to be unknown but there are certain predispositions that are strongly correlated with the incidence of PCOS. 02 Causes PCOs Hormones PCOS are found to have an imbalance in certain hormones, including: ❖ Raised levels of testosterone – a hormone often thought of as a male hormone, although all women usually produce small amounts of it ❖ Raised levels of luteinising hormone (LH) – this stimulates ovulation, but may have an abnormal effect on the ovaries if levels are too high ❖ levels of follicle stimulating hormone (FSH) in PCOS appear to be low or within the lower follicular range. ❖ Insulin resistance ❖ Insulin resistance and compensatory hyperinsulinemia are said to be significant causes of hyperandrogenism in women with PCOS. ❖ Furthermore, obesity worsens these hormonal imbalances thus making the clinical features evident - it has been observed that women with PCOS who are obese have a higher incidence of menstrual irregularities and hirsutism compared to non-obese women with PCOS POLYCYSTIC OVARY DIAGNOSIS NORMAL POLYCYSTIC OVARY OVARY Symptoms 03 Symptoms Of PCOs Infertility Obesity Recurrent first trimester miscarriages Dysfunctional Anovulation Uterine failure of the ovaries to release Bleeding an oocyte ACNE related to hyperandrogenism Diagosis 04 Diagnosis Hormonal Blood Test Ultrasound 1. Testing for androgen levels and free androgen index (FAI) is best for diagnosis hyperandrogenism. 2. Tests to detect female hormonal levels - Estradiol, Follicle Stimulating Hormone, Luteinizing Hormone levels Managment 05 Management 1- Infertility – pregnancy 2- Hirsutism Prior to any treatment exclude other causes of infertility (COC) Combined oral contraceptive piles.  In patient as tubal adhesion or Endometriosis Treatment  Medical :- Glucophage which decrease insulin resistance so it may broke down the cycle of PCOS and pregnancy may occur.  surgical:- Ovulation induction 4- Obesity 3-Menstrual irregularity  Artificial cycle through COC for 21 days fowled by stop 7 days  Hysterectomy in old patients. Recommendation of Exercise Training in Polycystic Ovarian Syndrome Exercise training Low caloric diet and session exercise Low caloric diet and antidiabetic drugs Low caloric diet and electro- lipolysis THANK YOU
