PULSE OXIMETRY Oximetry: measures (metry). oxygen (oxi). ❑ Pulse oximetry measures (metry)the oxygen (oxi) concentration in arterial blood. ❑ Oxygen concentration is the amount (percent [%]) of hemoglobin containing O2 An agency may use 1 of these terms: Pulse oximetry or pulse ox. O2saturation or O2 sat. SpO2 (saturation of peripheral oxygen). ❑ Saturation means to be filled. Peripheral relates to the surface. SpO2 measures the mount of hemoglobin near the surface of the skin that is filled with oxygen. ❑ The normal oxygen concentration range is between 95%and 100%. ❑ A sensor attach to a finger, earlobe ,nose or forehead. ❑ Light beams on 1 side of the sensor pass through tissues. A detector on the other side measures the amount of light passing through the tissues. With this information, the oximeter measures the O2concentration. ❑ For Children : The sensor is attached to sole of the foot, palm of the hand, toe, or earlobe. If the child moves a lot, the earlobe is a better site. Pulse oximetry sites ❑ Good blood flow to the site is needed. ❑ Avoid swollen sites and sites with skin breaks. Bright light, nail polish, non-natural nails, and movements affect measurements. ❑ For a good sensor site: ❖ Place a towel over the sensor to block bright lights. ❖ Remove nail polish or use another site. ❖ Do not use finger sites with non-natural nails. ❖ Use the earlobe if there are shivering, seizure, or tremor movements. ❖ Do not measure blood pressure on the side of a finger site. Blood pressure cuffs affect blood flow. ❑ Oxygen concentration is often measured with vital signs. ❑ The pulse rate may be shown on the pulse oximeter along with the oxygen concentration. ❑ Report and record measurements according to agency policy. ❑ Oximeter alarms are set for continuous monitoring. ❑ An alarm sounds if: ❖ O2concentration is low. ❖ The pulse rate is too fast or slow. ❖ Other problems occur. Pulse oximeter Advantages: ✓ Requires no special training to use. ✓ Noninvasive. ✓ Inexpensive. ✓ Continuous real-time measurements (point-of-care testing). ✓ Avoids pain/risks of arterial puncture. ✓ Standard of care during procedures requiring general anesthesia. ✓ Standard of care during procedural sedation and analgesia. ✓ Small, requires little space, portable. ✓ Can be used almost anywhere. ✓ Can be stand-alone equipment or incorporated into a multipurpose monitor. ✓ Easy to apply and use. Pulse oximeter limitations Having cold hands. Poor circulation. Vasoconstriction due to pathology or medication. Moving while taking your reading. Wearing nail polish (especially black, blue, or green). Having artificial nails. Experiencing an extremely low oxygen saturation (below 80%). Having thicker-than-normal skin.
