Tuesday October 8, 2024 Feelings of Going Into Labor: - Backache - Lightening (baby moves down) - Increased vaginal discharge - Bloody show - Energy burst “nesting” - ROM - Amniotic fluid is alkaline pH 6.5 to 7.5 MATERNAL LABOR ASSESSMENT - Review of prenatal record Leads to focused assessment (Based on patient condition or complaint) - Quick review of body systems from prenatal record - Vital signs, lung sounds, any complaint (other than labor) - Depending on labor status, complete labor admission assessment (mostly WNL) - If in active labor, may need to defer some parts of assessment - If not in labor, more focus on reason for admission to help drive assessment priorities ______________________________________________________________________________ Passenger is the fetus Presentation =breech Lie = how the baby is lying in the uterus Attitude =head position (is its nose upwards or downward toward the chin) Station =what station is the baby at (0 station halfway in halfway out, +5 would be the entire head out, anything less than 0 is when the baby is still making its way out of the uterus) —------------------------------------------------------------------------------------------------------------------ Station: 0 station halfway in halfway out, +5 would be the entire head out, anything less than 0 is when the baby is still making its way out of the uterus -Mild -cheek -Moderate -nose -Strong -forehead STAGES OF LABOR 1st stage (0-10 centimeters) - "0 - 3 = latent labor - 4 - 7 = active labor - 8 - 10 = transition 2nd stage- pushing the baby out (time it takes to push the baby out) 3rd stage - delivery of placenta 4th stage- stabilize maternal vital signs First stage - back, leg pain, visceral Second stage - fetal descent, burning, pressure Third stage - contraction type pain Fourth stage - perineum pain, tear, episiotomy
