FIELDS OF NURSING Part VI Learning Objectives After learning the concept, the students will be able: 01 To identify which fields of nursing is appropriate 02 To determine the qualification of nurses in accordance with the chosen field 03 To maintain professional integrity INTRODUCTION This topic is all about different fields of Nursing, maintaining a profession qualification, resigning as well as dismissal from a job Classification of fields of nursing 1. Hospital or Institutional Nursing 2. PHN or CHN 3. Private Duty or Special Duty Nursing 4. Industrial or Occupational Health Nursing 5. Nursing education 6. Military Nursing 7. School Health Nursing 8. Clinic Nursing 9. Independent Nursing Practice Hospital or Institutional Nursing - Comprises all the basic components of comprehensive patient care and family health - Concept of modern hospital where in-patient and out-patient care are continuous - Describes the goal of medical care in most general hospital Hospital or Institutional Nursing - Participates in all phases of patient care of the acutely ill, convalescing and ambulatory patient - Care patient in the hospital or in out-patient department and plans for the nursing care needs about to be discharge - performs nursing measures that will meet patient physical, emotional, social, and spiritual health needs Functions: - Assessment of patient needs - Planning and providing care whether personal care, rehabilitation, or health instructions - Participated in research and training - teaching auxiliary personal - Assisting in teaching and supervision of nursing and midwifery students Advantages of Staff Nursing - - There is always a supervisor whom one can consult if problems exist - Nurses are updated with new trends in medicine and in the nursing care of patients - Undergo rotation to different units and have a chance to determine the special area of choice before permanently assigned in one area Advantages of Staff Nursing - - Have an 8-hour or works 40-hour week duty with 2 days of rest away from duty - Provision for sick leave, holiday, and vacation with pay as per institutions policy - Chance to be promoted to higher position once qualified Advantages of Staff Nursing - - Salary increase are given periodically according to merit system , thereby increasing initiative and best efforts - Considered as important member of the health team in providing care to the patients - More staff development programs are available Disadvantages of Staff Nursing - - Great possibility of understaffing which may put nurses to put in overtime work and sacrifice some of their plans. This is true to the hospital where budget for personnel is limited - Because of the bulk of works no time to improve skills thru continuing education programs or if the hospital are far-flung, no continuing education programs are provided - Administrative problems and overwork may tend to dissatisfy the staff nurse Qualification of nursing Service Administrators - In Sec 29 RA 9173 specifies that a person occupying supervisory or managerial position must have the ff: - Be a RN in the Philippines - At least 2 years of experience in general nursing service administration - With BSN degree, at least 9 units in management and administration courses at graduate level - Member of good standing of the accredited professional organizations of nurses Qualification of nursing Service Administrators - In addition, for a person occupying a chief nurse or nursing director position they must have the ff: - At least 5 years of experience in supervisory or managerial position in nursing - With a Master’s degree major in Nursing Qualification of nursing Service Administrators - In addition, for a person occupying a chief nurse or nursing director position they must have the ff: - At least 5 years of experience in supervisory or managerial position in nursing - With a Master’s degree major in Nursing Note: Provided further that CN in Public health agencies with Masters degree in Public Health /CHN shall be given priority CN in Military hospital with Masters and completed the General Staff Course (GSC) and a 5-year experience PUBLIC HEALTH NURSING /CHN - PHN is also known as CHN - Emphasis is on health promotion and prevention of diseases rather than care of the sick - needs deeper involvement and close cooperation with all members of the health team such as physician, nurses, epidemiologist, dentist, sanitary inspectors, sanitary engineers, nutritionists, health educators, social workers, the community and barangay leaders Advantages of PHN - - Focus of nursing is on family and community health rather than on individual basis. Nurse see the total picture of family and community health - Gives a better perspective of the health condition of the community and the health programs conceived and implemented by the government, and to appreciate the nurses role in nation building - Maximizes efforts to improvise where there are no sufficient facilities, supplies, and equipment Advantages of PHN - - It enable the nurse to utilize various community resources and maximize coordination with other members of the health team - Focus of care is more on educational and preventive aspects and have the privilege of contributing to the program for healthy citizenry especially among the rural poor - Individuals, family, and communities are motivated to assume responsibility for their own health care Disadvantages of PHN - - Cases found in PHN are limited mostly to chronic and communicable diseases - There are more hazards in PHN than in hospital nursing such as exposure to elements such as inclement weather, heat of sun, rain, dog , snake bites, accidents and others - There are no fixed hours of works. He nurse may be called any time - Some claim that PHN is not as exciting nor as glamorous as hospital work PRIVARE DUTY NURSING - - Nurses in private practice are expected to be expert clinician as well as expert generalist - Use the title of PDN, Private Nurse Practitioner, Special Duty Nurse, or Private Duty Nurse Specialist - A RN who undertakes to give comprehensive nursing care to a client on a one-to-one ratio - An independent contractor - Care can be provided in the hospital or at home 2 categories of PDNurse - 1. GENERAL PDN - Assessment of physical condition and interpretation of significance of the findings as basis for planning the nursing care - Identification of emotional and social factors and relating to signs and symptoms observed - Application of scientific principles in the performance of nursing technique - Works with patients’ family so that they gain understanding of his illness and cooperate towards promoting early recovery of patient 2 categories of PDNurse - 1. GENERAL PDN - Utilization of laboratory and diagnostic tests in promoting progress of care and enhancing his own usefulness as a health teacher and counselor - Knowledge and recognition of pharmacological effects of drugs and medication, its implication, and actions - Interpretation of doctor’s order concerning medicine and treatment and communicating the same effectively to the patient and carrying them out promptly and accurately with understanding of causes and effects 2 categories of PDNurse - 2. PDN SPECIALIST - Skill in handling, operating, and monitoring other complicated devices - Skill in interpreting data gathered from EGC, EEG, laboratory diagnostic results and vital signs - Skill in observing signs and symptoms and their favorable or untoward significance in the progress of patent care - Promptness and adeptness in instituting appropriate nursing measures Qualification of PDNurse - 1. Be a registered nurse in the Philippines 2. With the following documents for the current year, - Professional Tax Receipt (PTR) - PRC Card - Residence certificate 3. at least 2 years of bedside nursing experience as a professional nurse in general hospital immediately prior to application Qualification of PDNurse - 4. be acceptable to the director of nursing service as suitable for orientation to do private duty nursing 5. be a full time PDN 6. preferably have undergone a critical care nursing course 7. be a certified IV nurse therapy by the Association of Nursing Science Administration of the Philippines (ANSAP) Requirements of PDNurse - 1. To undergo orientation in the hospital as conducted by the Nursing Service Office 2. With complete paraphernalia’s 3. Wear complete uniform, white blazer can be used if required by the hospital 4. With medical certificate from certified medical center or laboratory (CXray, CBC, UA, SA Requirements of PDNurse - 5. With 2-character references 6. To acquire, foe succeeding years, certificate of 3 seminars or CEP attended 7. To show certification as an IV therapist by th ANSAP Requirements of PDNurse - Note: PDN is expected to follow the rules and regulations of the hospital regarding nursing procedures use of equipment, supplies, MAR, charting, diets. Adhere to the code of ethics Advantages of Private Duty Practice - the chance to see life as it really is, to study human nature and to know at close range interesting people of different nationalities, cultures, religion, and status - make real friends thru close association with the patient and his family - chance to travel and see the world Advantages of Private Duty Practice - Ability to own time, adjust work and private life satisfactorily, and direct off-duty time into enjoyable leisure - chance to abreast with new knowledge and procedures which provide an exciting, stimulating experience, with infinite variety of problems to solve and personal satisfaction to be gleaned - the challenge of giving his best in providing care and having the satisfaction of seeing its results Disadvantages of Private Duty Practice - Create a situation in which the patients become total dependent on them - many PDN graduated so long ago ang have not upgraded their knowledge and skills - there are not enough nurses for evening and night, shifts, for holiday, weekends, and vacation periods - being a lone practitioner minimizes opportunity for developing good relationships with other hospital personnel Disadvantages of Private Duty Practice - Create some private duty nurses resent supervision by hospital staff - little assistance is given by the head nurse when the patient is critically ill and PDN needs help in giving treatment or changing pt’s position, sometimes goes without meal because no reliever is given when patient cannot leave alone - have very little or no participation in professional activities, if they ever do, they do so on their own time and expense Disadvantages of Private Duty Practice - nurses who are very experienced and well qualified receive the same fees as the new graduates - face problems in private practice being independent contractors and there is greater need for liability insurance - there are no retirement, insurance, and Medicare benefits unless the PDN provide these for themselves Occupational Health Nursing or Industrial - OHN is specialty practice that provides and delivers health care services to workers - The practices focuses on promotion, protection, and supervision of workers health within the context of safe and healthy work environment - Autonomous and makes independent nursing judgements in providing health services - OHN works alone in situation where they may have to give immediate care to patients with serious injury , they need to be fully informed about legal responsibilities Occupational Health Nursing or Industrial - The first consideration is the patient’s welfare - They must make use of whatever first aid and nursing measures - Emergency or more extreme measures may be used during life-and-death situations - Actions will be judged against the yardstick of reasonable prudent nursing practice - Requires special skills Occupational Health Nursing or Industrial - In addition, industrial nurses must know company policies on personnel, insurance benefits, sick leave, pay rates, health programs, medical matters, and records - Most OHN have to bargain individually for their salaries - Works only during day hours - Off duty on weekends most of the time - Nurses in large plants requires overtime works - There are usually seniority wage increases , pensions and insurances Occupational Health Nursing or Industrial Advantages - Wide variety of help for the beginners - Valuable literatures can be obtained from many sources - Can seek support from fellow practitioners Nursing Education - Career opportunities today are better - A chronic shortage of nurse educators in all nursing educations - Nursing Education Qualifications of Nurse Educators: - Be a registered Nurse in the Philippines - have at least 1 year of clinical practice in the field of specialization - Member of good standing professional organization - Be a holder of Masters Degree Nursing Education Personal qualities and special abilities: 1. Capability to promote interest in the subject they teach, stimulate the minds of the students, arouse enthusiasm, quicken imagination, and awaken ambition 2. Competence in the field they teach Nursing Education Personal qualities and special abilities: 3. Resourcefulness with infinite patience, understanding, confidence and perseverance, able to inspire the students to develop their fullest potential 4. Expertise in providing nursing care in the chosen field and be a role model Nursing Education Qualification of a Dean: - Masters Degree - At least 5 years of experience in teaching and supervising - Those teaching in Graduate program must possess a post-Masters program or Phd Nursing Education Advantage: - Salaries are higher than those working in the hospital - Salaries varied according to experience, field of specialization, and educational attainment Military Nursing (The Nurse Corps) Functions: - To meet the nursing needs of today’s patient in AFP medical facilities - To prepare each nurse corps officer for future assignments at a higher level of responsibility in different stations and general hospital in times of peace and war - To teach and train enlisted personnel who perform nursing functions under supervision Military Nursing (The Nurse Corps) Qualification for Commission in Reserve Force Nurse Corps: - BSN - RN - A Filipino Citizen - Single and never been married for both male and female, female candidates not to be found to have given birth to a living or still birth - Mentally and physically fit with pleasing personality and good moral character Military Nursing (The Nurse Corps) Qualification for Commission in Reserve Force Nurse Corps: - Skillful in: a. applying nursing process in meeting health nursing needs b. communicating and elating with others c. making sound and rational judgment Military Nursing (The Nurse Corps) Qualification for Commission in Reserve Force Nurse Corps: - Willing to work both peaceful and wartime conditions - Not more than 32 years old at the time of commissioned - Minimum of 62 inches for males as height requirement - And minimum of 60 inches for female Military Nursing (The Nurse Corps) Qualification for Commission in Regular Force Nurse Corps: - Pass the rigid screening and battery test - Not more than 26 years old at the time of regular commission - Minimum of 64 inches for males as height requirement - And minimum of 62 inches for female Military Nursing (The Nurse Corps) Qualification for General Duty Nurse: - Must have the rank of 2nd Lieutenant - Must have adequate knowledge of general nursing theory and practice , including basic knowledge about biological, social and medical sciences and their applications - Must have knowledge of new development the nursing field - Ability to demonstrate and perform comprehensive nursing care Military Nursing (The Nurse Corps) Privileges and Benefits of a Military nurse 1.High salary rate and allowances according to rank - as 2nd lieutenant, same base pay as officers of equivalent rank , incentive pay, subsistence allowance, quarters and clothing allowance - flight pay for flight nurse which is 50% of base pay - hazard pay which is 20% of base pay - cold weather clothing allowance - overseas pay when detailed to foreign countries o schooling or on missions Military Nursing (The Nurse Corps) Privileges and Benefits of a Military nurse 2.
