On the eMAR, identify which medication is due. - Check if medication is verified/acknowledged on eMAR (no un-verified icon present). - Open allergy tab (under patient profile) and check allergies and reactions. - Medication card: State the drug name, reason for the medication (does the patient required this med, is it appropriate, what is the outcome for the pt receiving the medication), classification, mechanism of action (MOA), **safe** dose range, onset, peak and duration, pre and post assessments, side effects. - Complete pre -- assessment and document pre -assessment (check chart): VS, labs etc. - Must be able to state: "**it is safe to give**" and **"it is within my scope"** - Wash hands. **Gather** your medication from the Omnicell or medication cart. - Compare the med to the eMAR following the 7 rights (patient, drug, dose route, time, reason & documentation). - Complete 2nd check using the 7 rights (patient, drug, dose route, time, reason & documentation). - Ensure you have a cup of water for patient to swallow medications. - Wash hands. Bring computer station & medication to the bedside. - Introduce self and title. Inform patient what you are doing. Place the patient in a sitting position (high fowlers). - Ensure patient is awake and able to swallow the medications whole or crushed. - **Click on blue Barcode Tab (top right-hand corner)** - **Scan** patient ID band and use two patient identifiers to confirm pt ID (name, DOB). - Ask about allergies and compare allergy band to information on the eMAR. - **Scan medication package (pop up window with pt ID and med information appears)** - Complete the 7 rights comparing medication package with information in pop up window. Once completed, hit save. (stays blue) - **Pour** medication into a cup. - Raise height of bed - State the name of the medication being given and explain purpose and action of the medication. - Provide pt education (if required). Ask "do you have any questions?" - **Give medication** to the patient. Ensure medication is swallowed. - Lower the bed before returning to computer station, maintain HOB high fowlers for 30 min. - Click on blue pencil icon under the correct time and complete documentation. - **Click save then verify** - Close the eMAR. - Let pt know when you will return. - Complete the 4 P's, ensure call bell is within reach. - Wash hands. - State when to return to complete post assessment. - Evaluate response/ any side effects to the medication. Verbalize post assessment and documentation in eMAR. - This is a general procedure that can be applied to all medication routes - Learners should add detail as it relates to their specific client and clinical site - Rather than a checklist, this is a framework to support clinical judgement. Although activities are displayed in a list, some may occur iteratively/cyclically as you work through clinical judgement. - The procedure demonstrates the full expression of the medication administration competency. - Specific term benchmarking will be identified in the Creighton Competency Evaluation Instrument \*TBD - Learners should draw from their course content and assignments related to pharmacology, medication calculations, nursing informatics/documentation, and other medication related knowledge when preparing to administer medications. - Learners are also encouraged to *practice* (within their scope) such as in open lab. - Learners should have completed all relevant assessments and data collection prior to medication administration. - Medications should be included in concept maps and show **relevant** links to disease processes, labs and diagnostics. - Learners to consider human and system factors that may contribute to medication errors (reduce distractions, symbols/acronyms, high-alert medication and process, hazardous meds and process, infection control, violence risk). - Learners to consider the need for inclusion of medication administration in PDP's. - Instructors should be mindful of when clinical questioning occurs, where optimal discussions should occur **prior to or post** preparation and administration process, leaving students to focus solely on safe prep and delivery. - Instructors to provide structured feedback that is objective and focused on developing competency, including a chance for the student to self-assess, and the instructor to provide both positive and constructive feedback. - Instructors to use a coaching approach to assist learners in reflection in-action and on-action. - Instructors to remind students that processes change but principles don't! - Meditech Expanse (login required): CLMM Steps for Med Admin infographic.pdf - Cerner:
