Dental Aspects Dental findings in liver disease Alcoholic cirrhosis – often poor OH, dental neglect, dental erosion from gastric reflux Sialosis/parotid enlargement Gingival enlargement if patient is taking ciclosporin after liver transplant Children needing liver transplants may have delayed tooth eruption and discoloured/hypoplastic teeth Dental neglect Neglect Poor OH Increase in caries Increase in inflammation/periodontal disease Lack of professional dental care Sialosis Gingival overgrowth Dental management Increased bleeding risk Risk of poor wound healing and infection Problems metabolising drugs Dental management Infection control – universal precautions, staff should be vaccinated against Hep B If liver transplant, patient will be taking life-long immunosuppressants, increasing risk of infection – may need antibiotic cover for invasive treatment Avoid elective dental treatment until six months after transplant What else do we need to consider?
