Drug dosage is especially important so be sure to check dosage calculations and do good assessment for side/adverse effects and signs of toxicity . 2. When discharging- make sure family has clear teaching on correct units of measurement. 3. Use the smallest syringe to increase Accuracy of dose given. 4. Family-centered care means to be sure to do thorough patient teaching and to practice safe medication administration. 5. Principle of atraumatic care means to decrease psychological/physical distress. THIS MEANS YOU SHOULD NOT separate PAITENTS FROM THE CAREGIVERS, AND YOU MUST support THE CAREGIVERS. GERIATRIC You will often have decreased dose due to aging organs, especially seen in medication administration with patients who have problems in the GI tract, renal, kidney, or cardiac issues. Polypharmacy is 5+ medications daily and the problem with this is that there is an increase in drug interactions and side effects. Table 7.2 very helpful for adherence. It is highly recommended to have the geriatric patient write down when meds are taken, Use an app, or utilize a pill organizer or container. Your goal is to promote autonomy. & keep the patient independent if possible. You should also always expect that they can learn.
