Newborn screening EXPANDED PROGRAM ON IMMUNIZATION EPI was established in 1976 to ensure that infants/childhood and mothers have access to routinely recommended infant/childhood vaccines 6 vaccine-preventable diseases were initially included in the EPI ТВ Poliomyelitis Diptheria -Tetanus Pertussis and Measles GOALS OF EPl and SUPPORTING LEGISLATION To achieve the overall EPl goal of reducíng the morbídity and mortality among children against the most vaccine-preventable diseases,the following laws have given the mandate of protecting children through immunization to the DOH and LGUS RA 10152, also known as Mandatory Infants and Children Health Immunization Act of 2011, Added to the six immunizable diseases previously mentioned are Hepatitis B Mumps Rubella Diseases caused by Haemophilus influenzae type B(Hib) Following Specific goals of the program (DOH, 2011): □ To immunize all infants/ children against the most common vaccine-preventable diseases o To sustain the polio-free status of the Philippines □ To eliminate measles infection a To eliminate maternal and neonatal tetanus o To control dipthería, pertussis, hepatitis B, and German measles □ To prevent extrapulmonary TB among children. INSERT PCI CONSIDERATIONS TO MAINTAIN POTENCY Observe the first expiry- first out (FEFO) policy Comply with recommended duration of storage and transport. At the health center/RHU with a refrigerator, the duration of storage should not exceed one month. Using transport boxes, vaccines can be kept only up to a maximum of 5 days. Take note if the vaccine container has a vaccine vial monitor (VVM) and act accordingly. Abide the open-vial policy of the DOH. A multidose vial may be opened for one or two clients if the health worker feels the client cannot come back for the scheduled immunization session For multidose liquid vaccines, maybe used in the next immunization provided the ALL the ff. conditions are met The expiry date has not passed the vaccine vial has not been The vaccine has not been contaminated. Submerged in water. The vials has been stored properly. INSERT PIC MAINTAIN THE COLD CHAIN At the RHU/ health centers the PHN act as a COLD CHAIN OFFICER -this means that the nurse is in charge of maintaining the cold chain equipment and supplies, -freezers, refrigerator, transport boX, vaccine bags/carriers, cold chain monitors, thermometers and cold packs COLD CHAIN REQUIREMNETS OPV\--15 to-25 deg.C. OPV has to be stored in the freezer In the vaccine bag OPV is placed in contact with cold packs All other vaccines, including measles,MMR, Rotavirus have to be stored in the refrigerator at a temp of+2 to +8 deg C. These vaccines should be stocked neatly on the shelves Hepatitis B vaccine, Pentavalent and TT are damaged by freezing so they should not be stored in the freezer. NEWBORN SCREENING NBS-is á simple peocedure to find out if a baby has a congenital metabolc d/o that may lead to mental retardation or even death if left UNTREATED \>Itis important also because most babies with metabolic d/o look normal at birth. R.A 9288 aka Newbom screening act of 2004. This law states that, prior to delivery, any health practitioner who delivers, or assists in the delivery, of a newborn in the Philippines has the obligation to inform the parents or legal guardian of the newborn of the avalabilty, nature, and benefits of NBS Ideally itis done on the 48h-72rd hours of life, NBS may also be done after 24 hours from birth The specimen for NBS is obtained through a heel prick. A few drops of blood are taken from the baby\'s heel, blotted on a special absorbent filter card and then sent to an NSC DISORDERS TESTED FOR NBS CONGENITAL HYPOTHYROIDISM-a condition in which the baby is bom with the nably to produce enough thyrold hormone CONGENITAL ADRENAL HYPERPLASIA-a group of inherited disorders characterized by the inability of the adrenal gland to secrete cortisol or aldosterone or both GALACTOSEMIA-an inherited d/o in which the body is unable to metabolize galactose and the person is unable to tolerate any form of milk-human or animal PHENYLKETONURIA-an inherited condition in which a baby is born without the ablit to properly breakdown an amino acid called Phenylalanine GLUCOSE 6-PHOSPHATE-DEHYDROGENASE DEFICIENCY (G6PD)-a hereditary condion in which red blood cells breakdown when the body is exposed to certain drugs, foods, severe stress, or severe infection MAPLE SYRUP URINE DISEASE-a genetic defect in which a person is unable to break down the amíno acids leucine, isoleucine, and valine: urine of affected persons smells like maple syrup NEWBORN HEARING SCREENING \>RA 9709, also known as the Universal Newborn Hearing Screening and Intervention Act of 2009 The law places on any health care practitioner who delivers or assists in the delivery of a baby in the Philippines the obligation to inform the parents or legal guardian of the newborn of the availability, nature and benefits of hearing loss screening among newborns or infants 3 months and below Parents and or legal quardians of newborns who refuse the test shall sign a waiver indicatng their understanding of the risks of undiagnosed congenital hearing loss Eary detection and intervention facilntate speech development and prevent future learning and psychosocial difficulties of the child with hearing impairment
