DAILY ROUTINE NEWBORN CARE first 24 hrs or 7 days goals: - establish and maintain homeostasis - stability of normal physiological status OBSERVATION * twice daily fir detection kf abnormalities ANTHROPOMETRIC * 2.9 kg - normal, variation: 2.5-3.9 WARM CHAIN * room temp - 25°C BREASTFEEDING * initiate bf 1 hr after birth to prevent hypoglycemia * proper techniques of bf * number one thing to ensure: proper latching signs of good attachment/ LATCHING ON - more areola is visible on top of mouth - baby’s mouth is wide open - lower lip is turned outward - correct positioning = effective sucking - after breastfeeding: BURP THE BABY TO PREVENT REGURGITATION preparing infant - ensure the infant is alert - baby’s head and body is straight - mother should support the baby’s whole body duration of breastfeeding * health newborn should feed ON DEMAND, whenever the baby cries for feed * interval: 2-3 hrs * crying - late sign of hunger * should feed 8-10 times for 24 hrs * should not skip night feeds - baby sleep well - passes urine 6-8 times(below that=dehydration) - crosses birth weight by 2 weeks - gains weight 25-30 g per day after initial 7 days (physiologic weight loss) BREASTMILK EXPRESSION/STORAGE * express in clean and dry area * thaw in luke warm water * once milk is thawed, should be used within 24 hrs/4hrs in ph * once milk is warmed and not used should be discarded * in room - 25°c - 6-8 hrs (maximum time to consume) * insulated thermal bag - 24 hrs * ref - 4°c - 5 days * freezer compartment - -14° - 2 weeks * freezer w seperate door - -18° - 3-6 mos * chest freezer - -20° - 6-12 mos COMPLICATIONS * breast pain and sire nipple (expected finding) * sore cracked nipples (portal if entry for mastitis organisms) - sore/cracked nipples causes
