Clinical Biochemistry of Liver Dr Mridula Chopra Introduction to Liver Learning Outcomes This session will cover basic liver functions and provide you with a summary of the routine liver function tests. After this session, you should know: Basic Liver functions Biochemical tests used to assess liver function/dysfunction. Liver o Size: Largest solid organ with large reserve capacity. o Fluid supply: o Has dual blood supply – hepatic artery, portal vein o Lymph vessels o Bile duct – transport and excrete bile from the liver into the gall bladder o Liver cells: 60% - Hepatocytes (functional cells), Kupffer cells, Endothelial cells, Stellate cells o Capable of regeneration. Summary: Liver functions  Metabolic: Metabolism of carbohydrates, fats and proteins  Synthesis: Proteins (including albumin, several enzymes), clotting factors, and complement proteins.  Storage: Glycogen, TGs, Iron, Copper, fat-soluble vitamins A, D and Vitamin B12.  Detoxification: Detoxification of foreign compounds such as drugs and endogenous substances like ammonia.  Bile Production: The main components are bile salts, bile pigments, and cholesterol What does Liver DO?.contd….. oEnzyme production: Aspartate aminotransferase (AST) Alanine amino transferase (ALT) Gamma-glutamyl Transferase (GGT) Alkaline phosphatase (ALP) Routine Liver function tests -1 Property being assessed Standard test Raised ALT and Hepatocyte damage AST, Conjugated bilirubin Biliary Damage/obstruction Raised ALP, GGT and conjugated bilirubin Albumin, protein, Urea, Synthetic Function Prothrombin time, Unconjugated bilirubin Liver function tests -2 Property being assessed Standard test Synthetic Function contd Raised lipids (Cholesterol, Triglyceride) Excretion/Detoxification Bilirubin, Ammonia Drugs and their metabolites End part 1
