Chapter 2 Caring and the Advance Practice Nurse CARING Caring is the essence of being human Nursing is a deeply human relationship Thus, caring is the essence of nursing Caring and Nursing  Caring and Human All human service disciplines Service Disciplines are based on caring Nursing, however, is unique in that caring is what most directly characterizes nursing’s knowledge base and service Medicine  Caring Is the MATRIX The “stuff” within which the A P R N-patient relationship is brought to life Knowing another as a caring person requires a commitment to entering into the world of the nursed with the explicit intention of knowing that person individually and uniquely APRN Practice Draws on and transforms characteristic medical ways of practice for nursing purposes Specialized opportunities for creating situations of care that call forth specialized patterns of caring Generalized Patterns of Caring  Courage Authentic presence Advocacy Knowing Commitment Patience Courage     Intentionally being with another in the fullness of one’s personhood Authentic Presence The caring communicated through authentic presence is the initiating and sustaining medium of nursing within the nursing situation Gadow—Existential Advocacy— self is brought into the situation as a full partner Advocacy When the nurse offers existential advocacy, the nursed feels truly known, respected, and connected in a way that affirms humanity and being “Knowing that” and “knowing about” refer to descriptions and analyses of the patient’s situation in the context of facts and information – empiric data points Knowing “Knowing directly” implies being deeply attuned; comes through intentional and authentic presence Patterns of Knowing  Practitioner draws on personal knowing as essential intuitive knowing Empiric knowing: avenue for drawing on science and skilled observation Ethical knowing: prompts consideration of values and morals for situations Patterns of Knowing (continued) Esthetic knowing: incorporation of knowing into a situation for integrated understanding with the nursed Commitment Choosing to be a member of the discipline and profession of nursing speaks to a lifelong commitment of service to humankind Commitment directs obligation or what “ought to be” in a particular situation Not experienced as a burden but as a response that is right, conscious, and caring Some challenges specific to A P R N practice Challenges to Economically driven care might conflict with calls for nursing from Commitment patients A P R N has unique opportunity to demonstrate how a commitment to the values of nursing influences the outcomes of care Commitment Choosing to be a member of the discipline and profession of nursing speaks to a lifelong commitment of service to humankind Commitment directs obligation or what “ought to be” in a particular situation Not experienced as a burden but as a response that is right, conscious, and caring Always occurs in a relational context Nursing as Human Calls for continued commitment Science to understand better the lived experience of the nursed, to truly hear their stories, and to respond in ways that matter, ways that nurture and sustain persons as they live and grow in caring Patience    Nursing Situations   Calls for Nursing What does your patient need right now?
