Infection/Immunity Case Study Scenario Name: Allie Ergy Age: 28 Gender: Female Medical History: No known allergies or chronic medical conditions Chief Complaint: "I'm having difficulty breathing, and my whole body is covered in hives. I feel really lightheaded and dizzy." Presenting Symptoms Difficulty breathing Generalized hives (urticaria) Swelling of the lips and tongue Dizziness and lightheadedness Anxiety and restlessness What do we think is going on with Allie? What type of hypersensitivity reaction is this? What immunoglobulins are associated with this? What other exposures are associated with this type of hypersensitivity reaction? Client’s Background Background Information: Allie Ergy, a 28-year-old female, presents to the emergency department (ED) with acute onset symptoms. She reports eating seafood for dinner about 30 minutes before the symptoms began. Allie states that she has never experienced anything like this before and is concerned about her current condition What do we think is the cause of the reaction? Assessment Findings What would you expect to find in your assessment? Tachycardia (heart rate: 120 bpm) Increased respiratory rate (32 breaths per minute) Blood pressure: 90/60 mmHg Oxygen saturation: 92% on room air Labored breathing with audible wheezing Periorbital and facial swelling Diffuse erythematous hives covering the entire body Nursing Diagnosis What could potential nursing diagnoses be for this client? Impaired Gas Exchange related to airway obstruction and respiratory distress. Risk for Ineffective Tissue Perfusion related to decreased blood pressure and potential shock. Anxiety related to perceived threat to life and unfamiliar symptoms. Plan of Care What are your priorities? Ensure patent airway Continuous cardiac and oxygen saturation monitoring Administer medications (epi-pen, steroids, antihistamine) Collaborative interventions Psychosocial interventions Discharge What would be an example of passive acquired immunity to help Allie in the future? The provider wants to slowly, and in a controlled environment, expose Allie to these allergens to prevent a future reaction. What type of immunity is this?
